30 dead, 139 sick from listeria in cantaloupe; a history of US food safety disasters

Before Al Gore invented the Internet in 1994, there was this thing called paper, which was useful for keeping records.

Those with a fetish in the macabre or statistics may care that the listeria-in-cantaloupe outbreak, which has now killed 30 and sickened 139, pales in comparison to past outbreaks.

Robert Tauxe, deputy director of the U.S. Centers for Disease Control and Prevention’s Division of Foodborne, Waterborne and Environmental Diseases told Elizabeth Weise of USA Today the deadliest documented foodborne illness outbreak in the United States was in the winter of 1924-1925, when typhoid in raw oysters from New York City killed approximately 150 people and sickened more than 1,500.

The second largest outbreak linked to food occurred in Boston in 1911. Then, streptococcus in raw, unpasteurized milk killed 48 people and sickened more than 2,000. The disease was described as "septic sore throat’ at the time, Tauxe says. Similar but smaller outbreaks like this one led to a national move to pasteurize milk in 1924 by the U.S. Public Health Service.

In 1922 in Portland, Ore., another outbreak of "septic sore throat" killed 22 people and sickened 487. That round of streptococcus was also linked to raw, unpasteurized milk.

And in 1919, an outbreak of botulism from olives put up in glass jars in California killed at least 15 people in three states. It resulted in a major change in how items were canned so that botulism would no longer be a problem.

But, Americans don’t want medical care like that practiced in 1919, nor should food production be rooted in some nostalgic past. Every death and illness from food is tragic, especially if preventable; what can be done to prevent this happening again? Telling consumers to wash cantaloupes in bleach is not a solution.

Walnuts suspected in Quebec E. coli death

Quebec’s Health Department confirmed Thursday that one person in the province has died of E. coli O157:H7 after possibly eating contaminated walnuts.

The person had pre-existing health problems, a ministry spokesperson told CBC News.

No other information about the individual was released.

So far, 13 cases of E. coli illness in Canada have been linked to contaminated walnuts distributed by Quebec-based Amira Enterprises.

Nine of the cases have been in Quebec. Two have been confirmed in Ontario along with two in New Brunswick.

Health Canada has been warning people to avoid eating shelled walnuts from bulk bins or certain brands, including Amira, Tia and Merit Selection (right, from CBC, delivery trucks parked outside the Montreal offices of Amira Enterprises).

There has been no additional information for three days. This is Canada, and what with their fourth federal election in seven years, expect the information to be flowing forward in super-duper slow motion.

Where did the poop nuts come from?

Sometimes, I write so fast I miss details.

While updating news on the E. coli O157:H7 outbreak in walnuts imported to Canada, I focused on the distributor, Amira Enterprises Inc. of St. Laurent, Quebec (that’s in Canada), and their website which stated they import specialty food products from Egypt, Lebanon, Syria, Turkey, Saudi Arabia and Iran.

What I glossed over was this statement: “The raw shelled walnuts are imported from the U.S.”

Where did the nuts originate within the U.S.? And where did the nuts get pooped on in the farm-to-fork system?

BTW, Amira, it would be courteous if you put some information on your own website about the walnuts you voluntarily recalled.

US to Canada: your meat inspection sorta sucks, only send us the good stuff

The dean of Canadian food and farm reporting, Jim Romahn, has written a powerful piece about the continuing failures in Canadian meat inspection – failures that had to be pointed out by Americans.

More than a year after 21 people died after eating Maple Leaf Foods Inc. products contaminated with Listeria monocytoges, the Canadian Food Inspection Agency was failing to enforce its own standards and there was sloppy follow-up when hazardous conditions were identified.

Those worrisome facts are contained in a report prepared by two U.S. inspectors who visited in the fall of 2009 to check Canada’s compliance with its own standards. They visited headquarters in Ottawa, 23 meat-processing plants and two labs.

They found that the Canadian Food Inspection Agency generally has good manuals and intentions, but falls short at the plant level, including failures to identify lax sanitation and to enforce its standards.

Thirty years ago, when Canadian reporters began to obtain U.S. inspection reports on our packing plants, all of the deficiencies identified applied to specific problems and individual plants. This audit has identified similar deficiencies at the plant level, but far more serious, it found deficiencies in the overall system.

Had the U.S. inspectors not checked it’s likely that the deficiencies would have persisted, putting Canadian consumers at risk and the meat industry under threat of losing export markets.

The report also indicates some of the systemic deficiencies were identified during previous annual audits, the Canadian Food Inspection Agency promised to fix them, but they persisted. This is after the Maple Leaf crisis and frequent promises by Agriculture Minister Gerry Ritz and Prime Minister Stephen Harper that corrective action would be taken as swiftly as possible.

They have never talked about the U.S. audit reports that highlighted things that needed attention.

During the inspections of 23 plants – Canadian officials went along with the two U.S. officials – they identified problems so serious that three plants were banned from marketing their products in the U.S. and three more were issued warnings that they would be banned if they failed to immediately correct deficiencies.
Four of these six plants were processing ready-to-eat meat products, meaning they would go directly to consumers without any further steps to eliminate hazardous bacteria.

They also found by checking records that some of the plants were running overtime hours without any government inspectors checking conditions.

At another plant, they said the documents that indicated compliance “did not consistently reflect the conditions encountered at the time of the audit.” Later in the report, they write “the actual conditions of the establishment visits were often not entirely consistent with the corresponding documentation.”

Supervisors are supposed to periodically check the performance of front-line government inspectors, but the auditors found “system weaknesses . . . in the manner in which supervisory reviews were conducted.”

They also said there was “an inconsistent identification of potential non-compliances or potential inadequate performance by the inspection personnel.

“The deficiency concerning the lack of supervisory documentation is a repeat finding from the 2008 audit,” this report says.

The two U.S. auditors say the Canadian Food Inspection Agency needs to improve its communications its employee training and awareness and its feedback systems.

They found inspectors were failing to do their duties, as outlined in agency manuals, because they noticed:

– “Lack/loss of consistent identification of contaminated product and product-contact surfaces and other insanitary (sic) conditions.
– “Inconsistent verification of adequate corrective actions . . . with regards to repetitive non-compliances.
– “Inconsistent and loss of documentation of non-compliances in a manner that reflects actual establishment conditions, and
– “Lack/loss of increased inspection activities when non-compliance is observed . . .”

They add that “many of these findings are closely related to those identified during the previous audit.”

They also “identified system weaknesses regarding implementation and verification of HACCP (Hazard Analysis Critical Control Points) systems within the CFIA (Canadian Food Inspection Agency).”

More specifically, they identified “inaccurate analyzing of hazards” for HACCP protocols, “Inadequate implementation of basic elements of the HACCP plan, including monitoring and ongoing verification procedures” and “inappropriate verification of corrective actions taken in response to deviations from the critical limit (for harmful bacteria)”

In addition, they identified lapses in recording instances of non-compliance, such as failures to enter problems in the record book, failures to identify the level of bacterial contamination ad failure to record the “actual times when the entries were made.” They also noted that border inspectors conducting spot checks of Canadian hamburger heading to customers in the U.S. found “several occurrences of zero-tolerance failures in addition to two positive results for E. coli O157:H7.”

Between Jan. 1 and Oct. 31, the U.S. border inspectors turned back 61 million pounds of Canadian meat, calling for repeat inspection by Canadians, and rejected 7,277 pounds that “involved food-safety concerns.” Labeling could be the problem with some of the shipments that were sent back.

In response to this audit, the Canadian Food Inspection Agency said it would increase inspections at 126 of the 190 plants certified to export to the U.S. There is no mention of what will happen at plants selling only to Canadians.

They say “supervisors are now required to accompany inspectors on a quarterly basis. . .”

“The sanitation task was re-structured to focus more on a global assessment of plant sanitation including more emphasis on Ready-to-Eat areas and equipment.”

The CFIA is also stepping up its training programs, its identification of critical control points for its HACCP protocols, its “Listeria related inspection tasks associated with operational/pre-operational sanitation, ventilation (e.g. condensation), building construction and maintenance of equipment.”

The CFIA also says “an electronic application is being developed to allow inspection staff access to historical data at the field level which will provide for more timely compliance decisions.”

When he was asked about the audit, Agriculture Minister Ritz said he has committed an additional $75 million to meat inspection since that audit was completed.

The results of the 2010 audit are not yet available from U.S. officials.

FDA chief says farms fell short on egg safety

The companies that have recalled more than half a billion eggs following a salmonella outbreak fell short of safety standards at their farms, U.S. Food and Drug Administration Commissioner Margaret Hamburg told CNN Sunday, adding

"There’s no question these farms involved in the recall were not operating with the standards of practice we consider responsible.”

She said "about 1,000" people have been sickened by a salmonella outbreak that federal regulators have traced back to two Iowa egg producers. One of those companies said Sunday it is "devastated" by the possible connection between its product and salmonella.

Hillandale Farms said it shared "a number of common suppliers" with Wright County Egg, including a company called Quality Egg, which provided feed and young birds.

Both Wright County and Quality Egg are owned by the DeCoster family, which has a string of agribusiness interests in the Midwest and Northeast. Those companies’ records have come under new scrutiny since the recalls were announced earlier this month.

The Washington Post reports that in the past 20 years, according to the public record, the DeCoster family operation, one of the 10 largest egg producers in the country, has withstood a string of reprimands, penalties and complaints about its performance in several states.

In June, for instance, the family agreed to pay a $34,675 fine stemming from allegations of animal cruelty against hens in its 5 million-bird Maine facility. An animal rights group used a hidden camera to document hens suffocating in garbage cans, twirled by their necks , kicked into manure pits to drown and hanging by their feet over conveyer belts.

And today’s USA Today reports that to some experts, the huge recall of potentially contaminated eggs is a testament to how the industry has grown from many small producers to large industrial farms.

The problem, many food safety experts say, is that even as eggs moved to a very intense production method with enormous companies and huge flocks, regulation was almost entirely lacking.

In 1999, President Clinton vowed to increase regulation and wipe out the disease in eggs by 2010. Instead, the industry and FDA delayed the creation of the rules, finally written in 2004.
 

Not just a recall, an outbreak: 7 sick with typhoid from frozen mamey pulp in shakes or smoothies

Associated Press reported today a rare U.S. outbreak of typhoid fever has been linked to a frozen tropical fruit product used to make smoothies.

Seven cases have been confirmed — three in California and four in Nevada. Two more California cases are being investigated. Five people were hospitalized, according to the Centers for Disease Control and Prevention.

The CDC said five of the victims drank milkshakes or smoothies made with frozen mamey fruit pulp. Four of them used pulp sold by Goya Foods Inc. of Secaucus, N.J.

Mamey is a sweet, reddish tropical fruit grown mainly in Central and South America. It is also known as zapote or sapote. It is peeled and mashed to make pulp, the CDC said.

The company has recalled packages of the pulp, sold in mostly western states. That press release said “no illnesses have been reported to date in connection with Goya brand Mamey Pulp.”

U. S. Customs and Border Protection officers stopped more than 100 pounds of soft Mexican cheese or queso fresco, hidden in false compartments of a vehicle trying to enter the United States across the Bridge of the Americas on Wednesday.

Associated Press notes federal officials permit travelers to import personal quantities of cheese — about 11 pounds per person.
