125 ill with norovirus at Oregon V.A. hospital

Retirement homes and hospitals have a lot of trouble with norovirus. If an ill resident, patient or staff member sheds the virus through vomit or poop in a public area a lot of folks can get sick.

According to KDRV ABC Channel 12, The V.A. hospital in White City Oregon is dealing with its own norovirus outbreak with over 125 veterans and 25 staff ill.vomit

“This is very contact oriented, it’s not airborne, it’s by touch,” said V.A. Infection Preventionist Sue Thurston.

Thurston said about 470 vets live at the V.A, and more than a quarter of them are sick.
Veterans are being asked to not leave their rooms until they feel better and bag meals are dropped off at their rooms.

“We’re wiping down everything you can touch –  all the side rails, all the doorknobs, all the vending  machines, all the rooms, all the surfaces, every single flat surface is being wiped down and disinfected,” said Thurston (I wonder what sanitizer they are using and wiping may just be spreading virus particles around -ben).

Although the virus isn’t respiratory, epidemiological investigations of past outbreaks suggest that virus particles can be aerosolized through vomit events. At IAFP 2013, North Carolina State University graduate student Grace Tung showed a simulated vomit event would yield a spread of droplets 8-12 ft.; the greatest distance traveled in any one experiment was 14.5 ft.

6 sick from chicken liver pate in Oregon, Washington

When I think Oregon, I tend not to think UK. But these regions are apparently bound by a passion for undercooked chiken liver pate resulting in Campylobacter outbreaks.

Since December 2013, Oregon health officials have been looking into the source of campylobacteriosis that has sickened six individuals in Oregon, Washington and Ohio. All cases report eating undercooked or raw chicken livers; most cases consumed chicken pate_beet_dp_mar_12livers prepared as pâté. The cases in Ohio ate chicken liver pâté while visiting Oregon. The Oregon Health Authority is working with the Washington Department of Health, USDA and CDC.

This is the second reported multistate outbreak of campylobacteriosis associated with consumption of undercooked chicken liver in the United States.

Australia has had its own outbreak.

Chicken livers should be considered a risky food. A recent study found up to 77 percent of chicken livers tested were positive for Campylobacter. Washing chicken livers is not enough; chicken livers can be contaminated on the inside and on the outside, which is why thorough cooking is the only way to kill bacteria in contaminated livers.

(Hint: don’t wash them, you’re just spreading Campylobacter around your kitchen.)

Pâté made with chicken liver is often undercooked to preserve texture. It can be difficult to tell if pâté is cooked thoroughly because livers are often partially cooked then blended with other ingredients and chilled. Pâté prepared at a USDA inspected facility is considered safe to eat because in order to pass inspection the livers must be cooked to a proper temperature.

The 2009 FDA Food Code states that restaurants must inform customers about the risk of eating undercooked food; the warnings are often included at the bottom of restaurant menus.

3 sick with Campylobacter in Oregon, Coos Bay Oyster recalls shucked and in-shell oysters

Lynne Terry writes that Coos Bay Oyster Co. is recalling oysters over a food poisoning outbreak that has sickened at least three people in Oregon.

The company, based in Charleston, said it is pulling all of its shucked oysters and in-shell oysters sold to retail stores and wholesalers in Oregon and California.

The shucked oysters were sold in 1/2 gallon, quart, pint and half-pint containers with sell-by dates from Jan. 15 to Feb. 17. The containers carry the Coos Bay Oyster Co. label and Coos Bay Oyster Co.are marked raw/ready-to-eat shucked oysters.

The oysters in-shell were distributed in red onion sacks, each containing five dozen oysters of various sizes. They, too, have the company’s label, with harvest dates from December 2013 to January 2014.

First cases of rare Salmonella strain in Oregon

The Oregon Health Authority Public Health Division confirmed Coos County had its first Salmonella Overschie case.

The victim was a 28-year-old female who became ill with Salmonella Overschie — a rare strain of the illness — in December 2013, said Lena Hawtin, clinical services supervisor for vomit_here_by_seedpix_at_flickrthe Coos County Public Health Department.

There was another case, a girl no older than nine, in Lane County, according to the report from OHA. This strain never has been reported in Oregon. There were 23 cases of Salmonella Overschie in the U.S. between 1995 and 2005.

A questionnaire analysis found both cases had shared exposures, including leafy greens, raw salsa, dog food, travel, etc., however the exact cause was unknown. The cases were a 99 percent match, indicating they likely came from the same source.

Illegal cheese source of Listeria for newborn in Oregon

Oregon public health officials issued a warning for illegally imported cheese from Mexico that is believed responsible for giving an unborn baby Listeria.

The infection was traced to unlabeled soft cheese called queso fresco sold in a Latino grocery store in Woodburn, officials said in a release. The listeriainvestigation by the Public Health Division and the Oregon Department of Agriculture found the cheese was brought into the country illegally from Mexico and sold in plastic bags.

A newborn baby became ill in late December, officials said, and believe the baby was infected before being born.

The guys started the show dreaming about a Red Mac Pro. They then turned to the passing of Bill Keene. Bill has been mentioned in various FST episodes and was a well respected epidemiologist as seen in the articles by the Oregonian and Doug Powell. The guys then turned to their beverages, Coffee Club, Napoleon Dynamite, Homeland, and Car Talk. Ben shared his preference for Aussie Rule football and Arcade Fire’s album Reflektor. The conversation then turned to Don’s limited iPhone music library, Privateering and Dire Straights, which reminded Ben of Money for Nothing and WWE Wrestling (not WWF Wrestling). Ben confused IAFP’s History with Bug Trivia and shared Julian Cox’s information about the 1960’s, and this evolved into a broader discussion about the IAFP and its membership.

The discussion then turned back to Bill Keene and some of the outbreaks he had been involved in. This included a Salmonella Panama outbreak (not to be confused with Van Halen’s Panama), which was the first outbreak that was solved through the innovative use of supermarket loyalty cards and that Bill and others were sued for (the lawsuit was eventually dropped.. The guys then discussed outbreak investigation in some detail and that public health officials are damned if they do and damned if they don’t name commodities and suppliers. There is of course always a risk of getting the epidemiology wrong, as was the case with Salmonella Saintpaul in peppers. Finally, Bill’s investigation of a Norovirus outbreak reminded Ben of a recent Norovirus outbreak in Las Vegas.

Then Ben commented on an exchange with Chris Gunter, who was presenting on traceability for small producers at the 2013 Strawberry Expo. Chris’ presentation is based on the investigation of an E. coli O157 outbreak related to strawberries, in which Bill Keene played a part.

Did mountain goats cause cryptosporidium outbreak in Oregon drinking water?

Dr. Bill Keene has cracked some novel outbreaks: deer poop on strawberries, norovirus on baby changing tables, salmonella in Guatemalan canteloupe.

Now, the senior state epidemiologist with the Oregon Health Division, is asking, were mountain goats the source of a cryptosporidium outbreak in Baker City, Oregon, that sickened at least 14 people and potentially hundreds?

The Baker City Herald reports that most of the confirmed crypto cases in the U.S. over the past 20 years — there were 7,656 confirmed or probable cases in 2009, and 8,951 in 2010 — were linked to sources such as swimming pools and daycare centers, Mountain Goat nanny and kidsnot municipal drinking water, according to the federal Centers for Disease Control and Prevention. For example, an outbreak of crypto that sickened at least 18 in Hull, UK, earlier this month was linked to a childrens’ splash pool.

If another theory about the Baker City outbreak pans out — that the crypto source was feces from mountain goats that live near Goodrich Reservoir — that would be the first such case in crypto annals, at least in the U.S., Keene said.

“I’m pretty sure mountain goats have never been tested (for crypto),” Keene said.

(Cougars or feral cat poop that entered drinking water are believed to have been the source of a toxoplasmosis outbreak in Victoria, B.C. in 1995 that sickened at least 110 and upwards of 3,000.)

Keene said crypto “has been around forever,” and that small numbers of oocysts likely are present in most surface water.

But given the rarity of outbreaks caused by municipal water supplies — including ones, like Baker City’s, that use unfiltered surface water — it seems that Baker City’s ordeal could be “the exception proving the rule,” he said.

Although federal statistics show a sharp increase in crypto cases starting in the 1980s, Keene said that trend likely reflects more widespread testing rather than an actual spread in crypto.

Until the 1980s crypto tests were rare, and they required a high level of expertise.

What brought the bug into the mainstream, so to speak, was the AIDS epidemic of the 1980s. Because AIDS patients have severely compromised immune systems they are especially vulnerable to crypto.

“People were dying from this,” Keene said.

Today, crypto tests are easier and more effective, and doctors and other officials are much more likely to suspect crypto than they used to be, he said.

Speaking of tests, Keene said the “official” count of 14 crypto cases in Baker City certainly doesn’t reflect the total number of infections.

That’s almost always the case in outbreaks, he said, because with otherwise healthy people the infection usually doesn’t cause symptoms that require medical attention, so relatively few people ever have stool samples tested.

Keene, who was in Baker City for several days before returning to his Portland office on Monday afternoon, said he and colleagues visited 21 Baker City homes and talked with 62 residents.

Of those, 18 had had symptoms consistent with crypto, Keene said — 29 percent of the total.

Keene said he wouldn’t conclude from that small sample size that one-third of residents were infected — that would be almost 3,000 people — but he said the total cases “could easily be in the hundreds.”

Rajneeshees: commune that purposely poisoned 750 diners in 1984 Oregon is now Christian youth camp

Mara Bovsun of the New York Daily News recounts the story of Bhagwan Shree Rajneesh, who breezed into rural Oregon in the early 1980s to spread love, enlightenment and, for those who did not believe, a little bit of salmonella.

Excerpts below:

On Sept. 17, 1984, the Wasco County health department fielded what seemed a routine call, a case of food poisoning after dinner at a restaurant in arts-graphics-2008_1130134athe town of The Dalles. It was nothing out of the ordinary; from time to time some bacterium makes its way into someone’s salad.

But this was different. The phones kept ringing with reports of people falling ill after eating in local restaurants. Within a week, the Centers for Disease Control pinpointed salmonella typhimurium. By then there were more than 750 cases in a town of 10,000.

CDC sleuths determined that the mass poisoning was not the result of poor food handling, but a deliberate attack by an invading army, clad in red, that had set up a base in Oregon three years earlier. They were known as the Rajneeshees, followers of the charismatic spiritual leader from India.

The man who would become known to the world as the Bhagwan Shree Rajneesh was born Chandra Mohan Jain in 1931, son of a cloth merchant from central India. When he was 7, the death of his grandfather traumatized him. As he grew up, he prided himself on never establishing attachments, which gave him the freedom to do whatever he wanted to anyone.

While he was in college his behavior became so bizarre that his parents tried to get him psychiatric help. Instead, in 1953, he became enlightened and found his life’s work — guru.

There, on a place formerly known as the Big Muddy ranch, the Baghwan built his American empire. It would eventually attract about 2,000 followers, dubbed “sannyasins,” all draped in the signature color of the cult — red — and wearing beaded necklaces with a picture pendant of their guru. Followers came from wealthy and elite circles, including Hollywood heavyweights and heiresses from such companies as Learjet and Baskin Robbins.

They paid generously for their path to fulfillment. The Bhagwan lived like a maharaja, with a fleet of 93 Rolls-Royces and piles of jewelry, mostly diamond-encrusted Rolex watches.

He lived in seclusion, emerging every so often to ride in one of his cars, and spoke to only one person, his trusted aide Ma Anand Sheela, 31.

The salmonella-in-the-salad-bar attack, is the largest act of bioterrorism on U.S. soil. That scheme was a practice run for a massive attempt to incapacitate Oregon voters by slipping bacteria into the water supply.

1 dead, 15 sick possibly from noro linked to fruit salad

KGW reports one person has died and 15 other people were sickened after a meeting in Forest Grove last Friday that may be linked to a norovirus outbreak, health officials said.

As first reported in the Oregonian, 43-year-old Kevin Weeks died Tuesday. Weeks, of McMinnville, was among 40 people to attend an Oregon Forests Advisory Committee meeting in Forest Grove.

Days after the meeting, Weeks and 15 other people who ate fruit salad at the meeting became sick with severe gastrointestinal issues, including vomiting and diarrhea, according to Tri-County Deputy Health officer Dr. Paul Lewis.  He added that the specific cause had not been determined but the symptoms and time frame were consistent with norovirus.

An autopsy was conducted Thursday and the State Medical Examiner’s office says Weeks’ cause of death was unknown, but did not appear to be norovirus. Lab results should be back in four to five days, and should reveal more.

The State Forests Advisory Committee met in the Forest Grove Community Auditorium. Breakfast and lunch were catered.

Weeks was a spokesman for the forestry department.
