Cause of Florida woman’s near-fatal food illness a mystery; exposes failures in system

The Palm Beach Post reports this morning that Amber Dycus, 38, of Loxahatchee, Florida, went to the hospital after four days of illness. The doctors told her she was in acute kidney failure, hours from death. She endured six days of intensive care, multiple blood transfusions and, so far, 196 bags of plasma.

There are more treatments to come, and no signs yet that her kidney function is approaching normal. She feels lucky to be alive, but also very afraid – afraid of eating out, afraid of catching germs, afraid of never getting better.

Dycus desperately wants to know what did this to her. Her lawyer, Craig Goldenfarb, thinks the public ought to feel the same way.

A health department inquiry has resulted in the brief closure of a Royal Palm Beach restaurant where Dycus often ate. Inspectors found roaches, improper food temperatures, slime in the freezer and a dishwasher with almost no sanitizer in it. After a thorough cleaning and a tuneup on the dishwasher, the restaurant, Hilary & Sons, has reopened.

But was it really the source of her illness? A series of missed opportunities, miscommunications, delays, and no small measure of scientific uncertainty means there may never be a conclusive answer.

At Palms West last month, Dycus was diagnosed with hemolytic-uremic syndrome. It’s an often fatal condition that happens when toxins cause red blood cells to shear apart and clog capillaries, shutting down the kidneys and leading to a buildup of waste in the blood.

It’s associated with outbreaks of dangerous E.coli O157 food poisoning.

Normally, when E.coli O157 is suspected, the health department is notified immediately, so that a public health investigation can be launched.

Dycus said her doctors told her she must have eaten contaminated beef. She’s grateful to them, and the nurses at Palms West, whom she says saved her life. But one thing they did not do was notify health authorities. A spokeswoman for Palms West said she could not comment.

It wasn’t until Dycus contacted a lawyer, and her lawyer called the media, that a health inquiry began. By then, a month had passed, the foods Dycus had eaten had long since disappeared, and the ability to tell exactly what sickened her had become nearly impossible to discern.

E. coli illnesses traced to raw milk from Minnesota farm

Minnesota Department of Health types have linked three cases of E. coli O157:H7 illness with raw milk from a dairy farm in Gibbon, Minn and are urging anyone who may have recently purchased milk from the Hartmann Dairy Farm, also known as M.O.M.’s, to discard the product and not consume it.

The milk may be labeled organic and consumers may be unaware that the milk has not been pasteurized. In addition, consumers should not eat cheese, ice cream or other dairy products from the farm, which also may have been made from raw, unpasteurized milk.

The Minnesota Department of Health and the Minnesota Department of Agriculture are continuing to investigate a cluster of four E. coli O157:H7 illnesses that all have the same “pulsed field gel electrophoresis” (PFGE) patterns, or DNA fingerprint. Three of the four cases report a link to milk from Hartmann Farm; the fourth case is under investigation. Three of the four people were hospitalized as a result of their illness; one case has developed HUS.

Fox9 reports that Wisconsin nearly passed a bill that would have loosened the restrictions on the sale of raw milk, but Gov. Jim Doyle vetoed the bill, stating,

"I recognize that there are strong feelings on both sides of this matter, but on balance, I must side with the interests of public health and the safety of the dairy industry."

Non-O157 E. coli outbreak at Ohio State University linked to University of Michigan outbreak?

In the interest of free and open discussion, Ohio State University types are being particularly tight-lipped about a possible E. coli outbreak, and possible links to an outbreak in Ann Arbor, Michigan.

Aren’t there a bunch of E. coli experts at both places?

I get that in the absence of information, it’s not a good idea to speculate. But these folks, based on their public writings, seem to know more than they are letting on.

And that puts others at risk.

Columbus Public Health is actively investigating an outbreak of food-related illness, caused by E. coli. Five cases of E.coli non – O157 have been reported to Public Health, two of which have been confirmed as a match to cases in a Michigan outbreak.

“We are currently working with all identified cases to collect the information we need,” says Dr. Mysheika LeMaile-Williams, Medical Director for the City of Columbus, “and we are actively doing everything we can to identify the source of infection.”

Those students got sick in mid-April.
 

Sorenne’s first Hepatitis A vaccination

Sorenne’s first birthday was fun but her 1-year doctor’s visit was not. She got 5 shots and a blood draw which left her in tears and a bit leery of nurses for future visits. All I could think about was how thankful I am that she is not a sick child and that this kind of torture is preventative and not curative treatment. I do not know how parents of sick children cope with watching their children suffer. Doug said, “Now imagine watching your child in the hospital with HUS.”

One of Sorenne’s jabs was for Hepatitis A. I got that shot and follow up in 2003-2004 because I was visiting risky countries like Iraq and Senegal. Sorenne will be immunized as a baby and we should never have to worry about at least that illness in the future. Now if only there were a vaccine for dangerous strains of E. coli, Salmonella, and other foodborne illnesses.

Three-year-old recovering from E coli but woman still in coma

A three-year-old girl who needed dialysis after being caught up in an E coli outbreak is beginning to recover in hospital, her parents said today.

Abigail Hussey suffered kidney failure after eating from a takeaway in Wrexham, north Wales, and is one of two people undergoing hospital treatment after the outbreak last month. Karen Morrisroe-Clutton, a new mother who also had kidney failure, remains in a medically induced coma at Wrexham Maelor hospital. The North East Wales NHS trust said she was in a serious but stable condtion.

She is in Alder Hey Children’s hospital in Liverpool, which today released a statement from her mother, Sarah, who also fell ill, and her father, Jeff.

"Abigail’s condition deteriorated and she was eventually referred to Wrexham hospital, who transferred her immediately to Alder Hey on Monday 27 July. She tested positive for E coli and was placed on dialysis. We are very relieved that Abigail is beginning to recover, is off dialysis and is eating and drinking quite well."

Sharon Mills, the mother of E. coli victim Mason Jones (left) said the latest Wales outbreak has brought horrific memories flooding back.

“It’s terrible that more people are having to go through this. Mason fought for two weeks until he couldn’t fight any more and ever since I have fought on for him as I don’t want his death to be in vain.”

While the cause of the North Wales outbreak remains under investigation, Mills said she believes both the authorities and the public still fail to fully appreciate the terrible consequences of E.coli infection.

The Llay Fish Bar was allowed to continue business even though environmental health inspectors found poor hygiene conditions and was awarded the lowest rating of no stars during the August 2008 inspection.

Mills said:

“The threat of E.coli is not being taken on board. People really need to start listening and they need to start listening now. The message needs to be drummed home that E.coli is serious and can affect anyone, not just those with underlying health problems. it’s such a powerful bacteria.”

 

E. coli cause of kidney failure in Iowa child?

KSFY is reporting that a one-year-old boy from Sioux Center, Iowa is in a Sioux Falls hospital tonight, fighting hemolytic uremic syndrome or HUS.

His dad told Action News today that Isaiah is normally an active kid, but this has slown him down and their time in the hospital has been heart-wrenching saying "you never think you’re going to see your child in the hospital and the reality of it is, is that it can happen to anyone. … It’s hard on us, but with God’s help we’re doing OK."
 

Child stricken with HUS in France; link to frozen ground beef patties

Our French correspondent Albert forwarded a press release issued yesterday by the French Ministry of Health and Sports; Amy translated.

Following notification on February 11, 2009 to InVS (The French Institute for Public Health Surveillance) of a case of hemolytic uremic syndrome in a child who is hospitalized in the Parisian region, health authorities have begun an investigation to identify the source of contamination.

The tests done on the child indicated he or she was infected with E. coli.

Among the foods consumed was frozen ground beef patties on which tests were conducted. The results of these tests were relayed today to health authorities and demonstrated the presence of E. coli. The link between this case and the consumed food will only be confirmed after further testing which is currently in progress.

While waiting for those results, the authorities have asked the producer to proceed with a  recall of CERGEL brand frozen ground beef patties sold in boxes of 10 with a best-by date of 10/31/09.

The health authorities are asking people who have bought these patties to not eat them and to return them to where they were purchased.

The Ministry also has some general advice, which seems a bit lacking, but maybe it got lost in translation.

Generally you are reminded that:

–    Ground meat ordered at the butcher shop must be consumed that day and frozen ground meat must be used without any prior defrosting;

–    Cooking the ground beef patties through to the center prevents the consequences of E. coli contamination. The bacteria is destroyed by a temperature of 65°C (149°F). Children and pregnant women should not consume rare meat.

–    Hemolytic Uremic Syndrome (HUS) is an illness most frequently originating in food, rare in France, and potentially serious for the very young and very old. It can cause acute renal insufficiency in children under the age of 3.
 

Over 200 sick with E. coli from Harvey’s; one child ‘very ill’ in hospital

The E. coli O157:H7 outbreak linked to a North Bay, Ontario, Harvey’s burger joint, is going from bad to worse.

The North Bay Parry Sound District Health Unit said today there are now a total of 207 cases, of which 39 are lab confirmed for E. coli O157:H7.

“Although we can reveal few details to avoid identifying anyone, there is one child who is very ill and in hospital,” said Dr. Catherine Whiting, Medical Officer of Health. “This person meets the criteria for complications from an E. coli infection, specifically Hemolytic Uremic Syndrome or HUS.”

The restaurant has been closed since Oct. 12. That’s more than enough time for DNA fingerprinting and to see if there are any matches with rather numerous E. coli outbreaks going on throughout North America. The CBC reports food samples have tested negative for the E. coli strain, and the Canadian Food Inspection Agency’s investigation has turned to testing food handlers. After only 11 days? Wow.  If this was an animal disease, CFIA would be all over it. But, it’s just people.
