Low levels of Salmonella may be associated with foodborne outbreaks

A significant data gap exists with respect to the levels of pathogens in foods implicated in foodborne outbreaks. These data are essential for the quantification of pathogen exposure via the ingestion of contaminated food.

Here we report the levels of the foodborne pathogen Salmonella in comminuted raw chicken products that had been breaded and then frozen. The products investigated were collected during four food safety investigations of foodborne outbreaks that occurred in Canada from 2014 to 2016. Most-probable-number (MPN) distribution analysis of the food samples revealed Salmonella levels of 0.0018 to 3 MPN/g, which is equivalent to 1 MPN per 0.33 to 556 g of product. These data suggest low levels of Salmonella may be associated with foodborne outbreaks.

Probably the lamb: 31 sickened with Salmonella in late 2014 at Sydney sports club

A foodborne illness outbreak involving an elite sports team was investigated by a public health unit in Sydney, Australia. An epidemiological association was established between gastrointestinal illness and the consumption of food supplied by an external caterer, with a lamb meal most strongly associated with illness.

Genetically identical Salmonella isolates were identified from clinical specimens, residual food items, and an environmental swab taken from the catering premises. The training schedule and other club operations were significantly affected by this outbreak. Increased susceptibility due to regular shared activities and the potential for significant impact upon performance indicates that sports clubs must ensure that food suppliers comply with the highest standards of hygiene. Collaboration with public health authorities assists in source identification and prevention of further transmission.

Citizen science: Using socks to sample environmental pathogens

This paper introduces a novel method for sampling pathogens in natural environments. It uses fabric boot socks worn over walkers’ shoes to allow the collection of composite samples over large areas. Wide-area sampling is better suited to studies focusing on human exposure to pathogens (e.g., recreational walking).

This sampling method is implemented using a citizen science approach: groups of three walkers wearing boot socks undertook one of six routes, 40 times over 16 months in the North West (NW) and East Anglian (EA) regions of England.

To validate this methodology, we report the successful implementation of this citizen science approach, the observation that Campylobacter bacteria were detected on 47% of boot socks, and the observation that multiple boot socks from individual walks produced consistent results. The findings indicate higher Campylobacter levels in the livestock-dominated NW than in EA (55.8% versus 38.6%). Seasonal differences in the presence of Campylobacter bacteria were found between the regions, with indications of winter peaks in both regions but a spring peak in the NW. The presence of Campylobacter bacteria on boot socks was negatively associated with ambient temperature (P = 0.011) and positively associated with precipitation (P < 0.001), results consistent with our understanding of Campylobacter survival and the probability of material adhering to boot socks. Campylobacter jejuni was the predominant species found; Campylobacter coli was largely restricted to the livestock-dominated NW. Source attribution analysis indicated that the potential source of C. jejuni was predominantly sheep in the NW and wild birds in EA but did not differ between peak and nonpeak periods of human incidence.

America too.

Marquette County in Michigan has recently experienced a small Salmonella outbreak among residents.

But that’s all that is being reported.

Coral Beach of Food Safety News called Patrick L. Jacuzzo, Marquette County director of environmental health, who described the outbreak as “restaurant associated” and said the assumption as of Friday afternoon was that raw, unpasteurized eggs served in Hollandaise sauce with eggs Benedict were the source.

So far, 8 people had been sickened.

Toddler showed ‘butthole’ during meal: Tennessee vegan

Dave Urbanski of The Blaze reports a customer named Chelsea Bartley wrote in 2-star review last week that she had a decidedly unappetizing experience at Imagine Vegan Cafe in Memphis, Tennessee — specifically that a “bare butt naked baby” with dirty feet “was running around, stood up on a table … and bent over to show me [her] butthole” during her meal.

That didn’t sit well with the cafe’s owner, who happens to be the mother of the 22-month-old girl in question. But not for the reason some might expect.

Kristie Jeffrey hopped on Imagine Vegan Cafe’s Facebook page, called out the customer by name and sent a warning to any other detractors.

“I’m about to start calling out names and pictures of people who leave us bad reviews, especially when it deals with our children,” Jeffrey wrote. “You will no longer be allowed to come and dine at Imagine. We do not need or want your business. … This is going to be fun!!!! You’ve throughly [sic] irritated mama bear!!!!!!!!!!!! We’re starting with Chelsea Bartley.”

Jeffrey added: “For anyone who reads this and instantly is scared this might affect our business, I cannot begin to tell you how much we do not care. Haters are not welcome at Imagine!!!!”

The post generated thousands of reactions, WMC-TV reported — but the station added that a few hours after its story ran about the dust-up, the post was deleted.

As of Friday afternoon, the cafe’s Facebook page appears to be down as well.

But Jeffrey did speak on camera to the station and didn’t back down from her position, noting that Imagine Vegan Cafe “has been a very family-oriented restaurant from day one. We’ve had crayons, kid menus, toys.”

And apparently poop.

In addition she told the station her four children are often in the restaurant during business hours. Jeffrey told WMC that while her daughter didn’t have a diaper on at the time of the incident noted in the 2-star review, she believes much of it is exaggerated.

“It was summer and it’s hot,” Jeffrey told the station about her daughter. “She does what a baby does, and she ripped it and she ran.”

And for my food, I can heat up a tofurky sausage just as well and in under half the time.

Jeffrey told WMC it would have been better if Bartley made her complaints known in person so it could have been handled, but the vegan cafe owner isn’t sorry about what the establishment stands for.

“I would actually rather not have their business, because it states it very clearly on our menus — on our website — this is what we are about,” she told the station. “If you can’t do vegan, then don’t come here. If you can’t do children, don’t come here.”

And if you can’t do basic microbiology and sanitation, expet customers to stay away.

Animal poop is everywhere in Bangladesh, and fecal indicator bacteria sorta suck

Fecal-oral pathogens are transmitted through complex, environmentally mediated pathways. Sanitation interventions that isolate human feces from the environment may reduce transmission but have shown limited impact on environmental contamination.

We conducted a study in rural Bangladesh to (1) quantify domestic fecal contamination in settings with high on-site sanitation coverage; (2) determine how domestic animals affect fecal contamination; and (3) assess how each environmental pathway affects others. We collected water, hand rinse, food, soil and fly samples from 608 households. We analyzed samples with IDEXX Quantitray for the most probable number (MPN) of E. coli.

We detected E. coli in source water (25%), stored water (77%), child hands (43%), food (58%), flies (50%), ponds (97%) and soil (95%). Soil had >120,000 mean MPN E. coli per gram. In compounds with vs. without animals, E. coli was higher by 0.54 log10 in soil, 0.40 log10 in stored water and 0.61 log10 in food (p<0.05). E. coli in stored water and food increased with increasing E. coli in soil, ponds, source water and hands.

We provide empirical evidence of fecal transmission in the domestic environment despite on-site sanitation. Animal feces contribute to fecal contamination, and fecal indicator bacteria do not strictly indicate human fecal contamination when animals are present.

634 sickened in national Salmonella outbreak, 2013-14, from one poultry company (Foster Farms)

In June 2012, the Oregon Health Authority and the Washington State Department of Health noted an increase in the number of Salmonella enterica serotype Heidelberg clinical isolates sharing an identical pulsed-field gel electrophoresis (PFGE) pattern. In 2004, this pattern had been linked to chicken from Foster Farms by the Washington State Department of Health; preliminary 2012 interviews with infected persons also indicated exposure to Foster Farms chicken.

In the end, at least 634 Americans were sickened with Salmonella Heidelberg, all linked to Foster Farms, from 2013-2014.

Here is the scientific write-up of that outbreak, or I’m completely wrong in my interpretation and will blame the painkillers.

Importance

This large outbreak of foodborne salmonellosis demonstrated the complexity of investigating outbreaks linked to poultry products. The outbreak also highlighted the importance of efforts to strengthen food safety policies related to Salmonella in the chicken parts and has implications for future changes within the poultry industry.

Objective

To investigate a large multistate outbreak of multidrug resistant Salmonella Heidelberg infections.

Design

Epidemiologic and laboratory investigations of patients infected with the outbreak strains of Salmonella  Heidelburg and traceback of possible food exposures.

Setting

United States. Outbreak period was March 1, 2013 through July 11, 2014

Patients

A case was defined as illness in a person infected with a laboratory-confirmed Salmonella Heidelburg with 1 of 7 outbreak pulsed-field gel electrophoresis (PFGE) Xbal pattern with illness onset from March 1, 2013 through July 11, 2014. A total of 634 case-patients were identified through passive surveillance; 200/528 (38%) were hospitalized, none died.

Results

Interviews were conducted with 435 case-patients interviewed with a focused questionnaire, 201 (74%) reported eating chicken prepared at home. Among case-patients with available brand information, 152 (87%) of 175 patients reported consuming Company A brand chicken. Antimicrobial susceptibility testing was completed on 69 clinical isolates collected from case-patients; 67% were drug resistant, including 24 isolates (35%) that were multidrug resistant. The source of Company A brand chicken consumed by case-patients was traced back to 3 California production establishments from which 6 of 7 outbreak strains were isolated.

Conclusions

Epidemiologic, laboratory, traceback, and environmental investigations conducted by local state, and federal public health and regulatory officials indicated that consumption of Company A chicken was the cause of this outbreak. The outbreak involved multiple PFGE patterns, a variety of chicken products, and 3 production establishments, suggesting a reservoir for contamination upstream from the production establishments. Sources of bacteria and genes responsible for resistance, such as farms providing birds for slaughter environmental reservoir on farms that raise chickens, might explain how multiple PFGE patterns were linked to chicken form 3 separate production establishments and many different poultry products.

Use a thermometer Ireland pt, deux: Growing trend for eating rare burgers could hide deadly bacteria

Gavin White of the Independent follows up on the warning from safefood Ireland that there is “no way of knowing” if rare burger meat is safe.

A leading food safety expert said he was “very surprised” restaurants were offering undercooked burgers and putting their customers at risk.

Professor Martin Cormican, from the school of Medicine in NUI Galway, said small children and pregnant women were at an even higher risk of becoming ill.

“Restaurants need to understand that not every customer is the same and some are at more risk than others. There are liability issues,” Prof Cormican said.

He said that every burger had the potential to have the deadly bacteria, Vtec, which could cause severe illness.

“Although steak can have its bacteria killed on the outside, mince has the potential for the bacteria to end up in the middle where if not cooked properly, has the potential to make you seriously ill,” he said.

Safefood Ireland has launched its Burger Fever campaign as it was revealed 96pc of Irish people consider themselves well informed about food safety, yet 51pc are eating undercooked burgers.

A batch of French mince was recalled last week from French supermarkets over worries for the presence of Vtec, and Prof Cormican said it could easily happen in Ireland.

“Don’t take the risk, and especially if you’re taking medicine for illnesses like rheumatoid arthritis which severely impacts your immune system,” Prof Cormican said.

Dr Linda Gordon, chief specialist in food science at Safefood, said around 2pc of all mince had Vtec in it so the risk was always there for the “growing trend” of burger lovers.

According to the U.S. Food and Drug Administration 2013 Model Food Code, it is the duty of a food establishment to disclose and remind consumers of risk when ordering undercooked food such as ground beef. The purpose of this study was to explore actual risk communication activities of food establishment servers. Secret shoppers visited restaurants (n=265) in seven geographic locations across the U.S., ordered medium rare burgers, and collected and coded risk information from chain and independent restaurant menus and from server responses. The majority of servers reported an unreliable method of doneness (77%) or other incorrect information (66%) related to burger doneness and safety. These results indicate major gaps in server knowledge and risk communication, and the current risk communication language in the Model Food Code does not sufficiently fill these gaps. Furthermore, should servers even be acting as risk communicators? There are numerous challenges associated with this practice including high turnover rates, limited education, and the high stress environment based on pleasing a customer. If it is determined that servers should be risk communicators, food establishment staff should be adequately equipped with consumer advisory messages that are accurate, audience-appropriate, and delivered in a professional manner so as to help their customers make more informed food safety decisions.

 

29 sick: An outbreak of Shiga toxin-producing Escherichia coli O121 infections associated with flour- Canada, 2016-2017

On December 29, 2016, PulseNet Canada identified a cluster of six Escherichia coli non-O157 isolates with a matching pulsed-field gel electrophoresis (PFGE) pattern combination that was new to the PulseNet Canada database. The patients resided in three geographically distinct provinces. In January 2017, the Public Health Agency of Canada (PHAC) initiated an investigation with local, provincial, and federal partners to investigate the source of the outbreak.

A case was defined as isolation of E. coli non-O157 with the outbreak PFGE pattern or closely related by whole genome sequencing (WGS) in a Canadian resident or visitor with onset of symptoms of gastroenteritis on or after November 1, 2016. Patients’ illness onset dates ranged from November 2016 to April 2017 (Figure). As of May 23, 2017, a total of 29 cases were identified in six provinces (Alberta, British Columbia, Newfoundland and Labrador, Ontario, Quebec, and Saskatchewan). One additional case was identified in a U.S. resident who traveled to Canada during the exposure period. Patients’ ages ranged from 2–79 years (median = 23.5 years) and 50% were female. Eight patients were hospitalized, and one developed hemolytic uremic syndrome. Clinical isolates were typed as E. coli O121:H19 (one case was typed as E. coli O121:H undetermined) with Shiga toxin 2–producing genes by in silico toxin testing and had closely related PFGE patterns and WGS.

Initial investigation into the source of the outbreak did not identify any clear hypotheses; common exposures were ground beef, sausage style deli-meats, pizza, and pork, but the data did not converge on any specific products. Patients were reinterviewed by PHAC using an open-ended approach. Knowledge of a recent E. coli O121 flour-associated outbreak prompted interviewers to ask about baking and exposure to raw flour or dough (1). Patients were also asked if any food items of interest, including flour, were available for testing.

In March 2017, E. coli O121 with the outbreak PFGE pattern was isolated from an open flour sample from a patient’s home and a closed sample collected at a retail store, both of the same brand and production date. The clinical and flour isolates grouped together, with only 0–6 whole genome multilocus sequence typing allele differences. As a result of these findings, a product recall was issued. Based on possible connections to the recalled lot of flour, market sampling of flour within certain periods was initiated. The investigation led to additional recalls of flour and many secondary products (2).

As of May 23, 2017, 22 patients had been asked about flour exposure in the 7 days before illness onset; 16 (73%) reported that the implicated brand of flour was used or probably used in the home during the exposure period. Comparison data on the expected proportion with exposure to this brand of flour were not available. Eleven of these sixteen patients reported they ate or probably ate raw dough during their exposure period.

This is the first national outbreak of non-O157 Shiga toxin–producing E. coli infections identified in Canada and the first Canadian outbreak linked to flour. An open-ended interview approach and flour sampling were used to implicate flour as the source. Because of the recent emergence of E. coli outbreaks linked to flour, public health professionals should consider flour as a possible source in E. coli outbreaks and communicate the risk associated with exposure to flour, raw batter, and dough in public health messaging.
