It would be interesting to find out how the glass may have made its’ way into the food product. What was the root cause that led to the possible contamination?

Loblaw Companies Limited has recalled one specific lot code of PC brand Sweet Chipotle Prepared Mustard from the marketplace due to the possible presence of glass.
Consumers should not consume the recalled product described below.

Check to see if you have recalled product in your home. Recalled products should be thrown out or returned to the store where they were purchased.
This recall was triggered by the company. The Canadian Food Inspection Agency (CFIA) is conducting a food safety investigation, which may lead to the recall of other products. 

 

Where did the hog come from?

Who knows….. I have run in to similar problems when I was in the field and it is incredible the stories people make up when you ask for the origin of products. If it is not from an approved source, just say it and get it out of your restaurant.

A whole hog of questionable origins and more than 100 live roaches led inspectors in December to temporarily halt operations for the second time at a Panama City Beach restaurant, according to health inspection reports.
It was the only restaurant reported to have been issued an emergency closure in the past month.
In December, the Florida Department of Business and Professional Regulations (DBPR) issued an emergency closure for Cool Runnings Caribbean Cuisine, 13312 Front Beach Road in Panama City Beach. Sanitation and safety specialists reported finding conditions that could contribute directly to a food-borne illness or injury at the time of their inspection. Cool Runnings, however, corrected the issues that led to the closure and was allowed to reopen within a day, state inspectors reported.
DBPR specifies the inspections are snapshots of a business at that time only. Cool Runnings has an active state licenses and is currently open. It is, however, the second time within the past few months the business has been temporarily shuttered for health violations, according to DBPR reports.
On Dec. 5, inspectors reported arriving at Cool Runnings about 2 p.m. to find a whole hog being stored in a reach-in freezer. After the restaurant was unable to provide an invoice or receipt to show the hog’s origins, inspectors issued a “stop sale” order on the food item, DBPR reported.
Inspectors also reported finding more than 100 live roaches in various areas of the kitchen.
“Observed 75 live roaches on the shelf next to root beer reach in cooler, in the back prep area,” inspectors wrote. “Observed 16 live roaches on the wall by root beer reach-in cooler. Upon eight live roaches underneath the prep table in back prep area. Observed five live roaches behind two-door reach in cooler in back prep area. Observed four live roaches underneath the three compartment sink in back prep area.”
Management of Cool Runnings did not return a request for comment on the closure. DBPR reported the business corrected the issue and was allowed to reopen the following morning about 10:30 a.m.
In August, inspectors reported finding flying insects in the restaurant’s kitchen, food preparation area and food storage area. In addition to the business operating with an expired DBPR license, officials also reported finding about 75 live roaches in areas near the restaurant’s hot water heater, underneath a reach-in cooler and around an umbrella during that visit.
That was the first emergency closure issued to the business since it opened in February, DBPR records indicated.
The closure brings the total to 21 for 2017 in the central Panhandle. The bulk of the closures occurred since the end of June after a two-month stint during which DBPR went without a closure.

Confused consumers: Canadians say E. coli in romaine outbreak is over; U.S. says it’s leafy greens

Outbreaks of foodborne illness are fraught with uncertainties.

It’s OK to admit, to do the best with the info available, and get on with things.

On January 10, 2018, the Public Health Agency of Canada reported that an outbreak of Shiga toxin-producing E. coli O157:H7 infections (STEC O157:H7) they had identified was linked to romaine lettuce appears to be over.

As of January 10, 2018, there were 42 cases of E. coli O157 illness reported in five eastern provinces. Individuals became sick in November and early December 2017. Seventeen individuals were hospitalized. One individual died.

In the United States, the Centers for Disease Control, several states, and the U.S. Food and Drug Administration continue to investigate a multistate outbreak of 24 STEC O157:H7 infections in 15 states. Since CDC’s initial media statement on December 28, seven more illnesses have been added to this investigation. The last reported illness started on December 12, 2017.

The likely source of the outbreak in the United States appears to be leafy greens, but officials have not specifically identified a type of leafy greens eaten by people who became ill.  Leafy greens typically have a short shelf life, and since the last illness started a month ago, it is likely that contaminated leafy greens linked to this outbreak are no longer available for sale. Canada identified romaine lettuce as the source of illnesses there, but the source of the romaine lettuce or where it became contaminated is unknown.

Whole genome sequencing (WGS) showed that the STEC O157:H7 strain from ill people in the United States is closely related genetically to the STEC O157:H7 strain from ill people in Canada. WGS data alone are not sufficient to prove a link; health officials rely on other sources of data, such as interviews from ill people, to support the WGS link. This investigation is ongoing. Because CDC has not identified a specific type of leafy greens linked to the U.S. infections, and because of the short shelf life of leafy greens, CDC is not recommending that U.S. residents avoid any particular food at this time.

In the United States, a total of 24 STEC O157:H7 infections have been reported. Among the 18 ill people for whom CDC has information, nine were hospitalized, including one person in California who died. Two people developed hemolytic uremic syndrome, a type of kidney failure.

The Public Health Agency of Canada identified romaine lettuce as the source of the outbreak in Canada. In the United States, the likely source of the outbreak appears to be leafy greens, but health officials have not identified a specific type of leafy greens that sick people ate in common.

State and local public health officials continue to interview sick people in the United States to determine what they ate in the week before their illness started. Of 13 people interviewed, all 13 reported eating leafy greens. Five (56%) of nine ill people specifically reported eating romaine lettuce. This percentage was not significantly higher than results from a survey of healthy people in which 46% reported eating romaine lettuce in the week before they were interviewed.  Based on this information, U.S. health officials concluded that ill people in this outbreak were not more likely than healthy people to have eaten romaine lettuce.  Ill people also reported eating different types and brands of romaine lettuce. Currently, no common supplier, distributor, or retailer of leafy greens has been identified as a possible source of the outbreak. CDC continues to work with regulatory partners in several states, at the U.S. Food and Drug Administration, and the Canadian Food Inspection Agency to identify the source.

Although the most recent illness started on December 12, there is a delay between when someone gets sick and when the illness is reported to CDC. For STEC O157:H7 infections, this period can be two to three weeks. Holidays can increase this delay. Because of these reporting delays, more time is needed before CDC can say the outbreak in the United Stated is over. This investigation is ongoing.

Looks like Chile has an egg problem: 174 sick with Salmonella linked to homemade mayo

The Bío Bío Department of Health has confirmed 174 cases of salmonellosis in people who consumed homemade mayonnaise in the local “Dulce y Salado” (“Sweet and Salty”) of Lota.

Among those affected are 25 hospitalized for severe dehydration, including a pregnant woman. There are also sufferers of all ages, such as a one-year-old baby and up to an adult older than 91.
According to the health authority, the number of patients should not increase substantially due to the number of days that have passed since the closure of the premises, on Jan. 3. 2018. Now it is expected that laboratories in Santiago will determine the serotype of the strain, which may allow them to fully define whether the raw egg was responsible for this outbreak.

 

Identifying foodborne outbreaks using social media

As a new survey shows 95% of chefs cite customers getting sick as their top concern, a computer system developed by Columbia University with Health Department epidemiologists can detect foodborne illness and outbreaks in New York City restaurants based on keywords in Yelp reviews.

Using Yelp, 311, and reports from health care providers, the Health Department has identified and investigated approximately 28,000 complaints of suspected foodborne illness overall since 2012 and helped Health Department staff identify approximately 1,500 complaints of foodborne illness in NYC each year, for a total of 8,523 since July 2012.

Improvements to the computer system are the subject of a joint study published this week by the Journal of the American Medical Informatics Association. The Health Department and Columbia continue to expand the system to include other social media sources, such as Twitter, which was added to the system in November 2016. The computer system allows the Health Department to investigate incidents and outbreaks that might otherwise go undetected. New Yorkers are encouraged to call 311 to report any suspected foodborne illness.

“Working with our partners at Columbia University, the Health Department continues to expand its foodborne illness surveillance capabilities,” said Health Commissioner Dr. Mary T. Bassett. “Today we not only look at complaints from 311, but we also monitor online sites and social media. I look forward to working with Columbia University on future efforts to build on this system. The Health Department follows up on all reports of foodborne illness – whether it is reported to 311 or Yelp.”

Each year,

“Effective information extraction regarding foodborne illness from social media is of high importance–online restaurant review sites are popular and many people are more likely to discuss food poisoning incidents in such sites than on official government channels,” said Luis Gravano and Daniel Hsu, who are coauthors of the study and professors of Computer Science at Columbia Engineering. “Using machine learning has already had a significant impact on the detection of outbreaks of foodborne illnesses.”

“The collaboration with Columbia University to identify reports of food poisoning in social media is crucial to improve foodborne illness outbreak detection efforts in New York City,” said Health Department epidemiologists Vasudha Reddy and Katelynn Devinney, who are coauthors of the publication. “The incorporation of new data sources allows us to detect outbreaks that may not have been reported and for the earlier identification of outbreaks to prevent more New Yorkers from becoming sick.”

“I applaud DOHMH Commissioner Bassett for embracing the role that crowdsourcing technology can play in identifying outbreaks of foodborne illness. Public health must be forward-thinking in its approach to triaging both everyday and acute medical concerns,” said Brooklyn Borough President Eric Adams.

Most restaurant-associated outbreaks are identified through the Health Department’s complaint system, which includes 311, Yelp, and reports from health care providers. Since 2012, the Department has identified and investigated approximately 28,000 suspected complaints of foodborne illness overall. The Health Department reviews and investigates all complaints of suspected foodborne illness in New York City.

Everyone’s got a camera: NYC-market-worker-boot-on-fish edition

Ciara McCarthy of Patch reports a Chinatown fish market has become the latest Internet sensation. A video filmed by a customer shows a worker climbing on a tray of fish, apparently to reach an electrical box. His boots are seen on top of the fresh produce.

The video was shot at the Hung Fee Food Market, located at 214 Canal St., on Jan. 3 and uploaded to Facebook the next day by April Davidson, she told Patch in a message. It had been viewed nearly 180,000 times as of Tuesday.

“Seriously, seriously, you’re gonna stand there on the food with your boots?” Davidson can be heard asking in the video.

The footage has garnered thousands of shares on Facebook and has spurred hundreds of Yelp reviewers to leave negative comments on the market’s page.

A woman who answered the phone at the fish market said she didn’t speak English and there wasn’t anyone available to comment.

An employee at the store told Pix 11 that the man had been called in to fix an emergency electrical problem and said that all the fish that were stepped on were thrown out before they could be sold.

The New York Department of Agriculture and Markets found “critical deficiencies” at the food market after conducting a review on Monday, spokeswoman Lisa Koumjian told Patch. The state agency dispatched a food safety inspector after seeing the video.

The Hung Kee Food Market was assigned a “C” grade for its sanitary conditions, Koumjian said. The state will conduct a follow-up inspection at the market in the near future. 

“The food safety inspector also addressed the consumer complaint regarding an individual standing on fresh fish,” Koumjian said in a statement.

“Management stated that a contractor, who was hired to repair an emergency electrical issue, stood directly on a fish display to access the electrical panel. Management has since instructed employees on proper access for electrical panels without jeopardizing product integrity and wholesomeness.”

Some Yelp reviewers came to the shop’s defense in the wake of the internet backlash. 

One man, who identified himself on Yelp as “Sailor J.” wrote, “Ok, I’ve been on the ocean for 30+ years… Have ANY of you freaksouts (who’ve likely never even been to this market) ever seen the deck of a typical commercial fishing boat when operating in full gear?” he wrote in his review. “The bottom of this guys shoes is, by far, not the nastiest thing these fish have touched.”

14 sickened in 2016: UK steakhouse fined 50K for serving food on wooden boards

A restaurant has been slapped with an $86,000 fine for serving food on wooden boards after 14 people allegedly suffered food poisoning.

Ibrahim’s Grill and Steakhouse in Birmingham was handed the fine last week.

Birmingham City Council tweeted health inspectors had been called to the outlet in October 2016 amid claims 14 people suffered food poisoning.

Along with disposable gloves being used instead of hand washing, authorities noted that wooden plates “incapable of being cleaned” were used to serve food.

“On a return visit in December 2016…wooden plates were still being used,” the council posted to social media on January 6.

Ibrahim’s Grill and Steakhouse, which maintains a 4-star rating on TripAdvisor, pleaded guilty to the fine at the Birmingham Magistrates Court on January 4.

Along with the 50,000 (A$86,000) fine, the restaurant will be forced to pay A$1158 in costs and A$200 “victim surcharge”.

“It’s completely unacceptable for businesses to put the health of people eating at their restaurant at risk,” Birmingham Head of Environmental Health Mark Croxford said in an official statement.

Whole genone sequencing fingers ham as source of Salmonella outbreak in Netherlands

In January 2017, an increase in reported Salmonella enterica serotype Bovismorbificans cases in the Netherlands was observed since October 2016. We implemented a case–control study to identify the source, including all cases after December 2016.

Adjusted odds ratios were calculated using logistic regression analysis. We traced back the distribution chain of suspected food items and sampled them for microbiological analysis. Human and food isolates were sequenced using whole genome sequencing (WGS).

From October 2016 to March 2017, 54 S. Bovismorbificans cases were identified. Sequencing indicated that all were infected with identical strains. Twenty-four cases and 37 controls participated in the study. Cases were more likely to have consumed ham products than controls (aOR = 13; 95% CI: 2.0–77) and to have shopped at a supermarket chain (aOR = 7; 95% CI: 1.3–38).

Trace-back investigations led to a Belgian meat processor: one retail ham sample originating from this processor tested positive for S. Bovismorbificans and matched the outbreak strain by WGS. All ham products related to the same batch were removed from the market to prevent further cases. This investigation illustrates the importance of laboratory surveillance for all Salmonella serotypes and the usefulness of WGS in an outbreak investigation.

61 dead from listeriosis in South Africa as fatalities double in a month

Wendy Knowler of Business Day reports the death toll from the listeriosis outbreak plaguing SA has nearly doubled in the past month, to 61 from 36, as SA grapples with an outbreak that experts say is the worst on record, worldwide.

The origin of the outbreak remains a mystery, though researchers have confirmed it probably has a single source.

Nonetheless, a Sovereign Foods abattoir has been closed, after listeria bacteria were found there.

Sovereign Foods, which is based in the Eastern Cape‚ is one of the major poultry producers in Africa. The company delisted from the JSE on November 22, concluding a management buyout funded by private equity firm Capitalworks.

But Health Minister Aaron Motsoaledi said the strain found at the abattoir was not the ST6 strain responsible for the deadly outbreak.

He told a briefing in Pretoria on Monday that a chicken sample collected from the fridge at a patient’s home tested positive for Listeria monocytogenes.

This chicken was traced back to the store and then traced back to the abattoir. It was sourced from Sovereign Foods‚ he said.

However‚ all samples collected from the abattoir have so far failed to pick up the ST6 strain of the outbreak that the country is experiencing.

As a consequence‚ authorities cannot yet link clinical isolates obtained from patients to particular foodstuffs or a food production site.

Motsoaledi said that the number of cases of listeriosis confirmed via lab testing had increased from 557 in early December to 727 at the latest count.

Food scientists are now calling it the worst documented listeriosis outbreak in global history.

The minister said 91% of the isolates were ST6 type isolates, a finding that supported the hypothesis that a single source of food contamination may have caused the outbreak from one or more food products at a single facility.

Dr Lucia Anelich‚ a prominent South African food microbiologist and food safety expert, said “I concur with my colleagues from business‚ academia and governments‚ in Europe‚ Australia‚ Canada and the US‚ that this is the worst documented listeriosis outbreak in global history.”
