48 million cases of foodborne illness a year; most are not linked to foul play

Foodborne illness happens. It sucks when it does.

It’s pretty much never intentional; not never, but rare.

In 1986, two Rajneeshee commune mem­bers were indicted for conspiring to tamper with consumer products by poi­soning food after over 750 community members in The Dalles, Oregon became ill with salmonellosis in 1982.

It sucks that twelve Alabamans ended up with what looks like foodborne illness after a holiday party this week. It’s weird that the Montgomery Advertiser coverage twice says that the cases probably weren’t as result of intentional contamination.

No foul play is suspected, and it looks like it is a case of accidental food poisoning, said Capt. Jeff Hassell, who commands the Prattville Police Department’s investigations division. Kinedyne Corporation, which operates a plant in the 1100 block of Washington Ferry Road, held its holiday lunch Friday. About an hour after eating, several employees complained of feeling sick, Hassell said.

Three employees were taken by ambulance to Prattville Baptist Hospital’s emergency room, with a fourth employee going by private vehicle, said Ernie Baggett, director of the Autauga County Emergency Management Agency. 

“We are investigating because it is an unusual situation, so many people becoming sick so quickly,” Hassell said. “Right now, we have nothing to point to an intentional act. We are looking at improperly cooked chicken as the most likely source for a food poisoning situation.”

“It was a pot luck dinner,” Baggett said. “No one became seriously ill, but a few employees wanted to go to the hospital just to get checked out.

 

Are chefs trying to kill us? (Asks the Boston Globe). Probably not. But might not be focused on public health

According to the Boston Globe, Today’s menus are filled with foraged food, fermented food, food that bubbles, food that molds, food that looks almost exactly like another thing that should probably not be called food because it is poisonous. It’s all perfectly safe, when sourced and prepared properly, under sanitary conditions, by people who adhere to proper procedure and take rules seriously. (Note: This last does not necessarily always describe chefs.)
“You shouldn’t be fermenting and jarring everything without a HACCP plan,” says Brandon Baltzley, chef and co-owner of Falmouth’s Buffalo Jump and a forager for Poplar. “But people know how to get away with it. Every place has a closet behind lock and key that has a lot of that kind of stuff in it.” He once visited a restaurant in Ohio that had a fermentation lab in a hidden attic that was just as big as the production kitchen itself, he says; a restaurant in another country had an entire secret facility a few blocks from the restaurant. 

When he cooked at Ribelle, a Brookline restaurant that has since closed, he would sometimes bring foraged ingredients into the kitchen. Several chefs, under condition of anonymity, reported it is easy to find workarounds when it comes to foraging. One recounted bringing a haul of mushrooms to a wholesaler, who then “sold” them back to the chef with appropriate documentation for a nominal fee. Another, appreciative of the flavor of wild clams from a particular area, purchased other clams, used their tags on the wild shellfish, and served the purchased ones for staff meal. The wild clams went to the customers.

Regulations, can sometimes be burdensome on the regulated party. Especially they aren’t familiar with the consequences. States set restaurant food safety laws, based on the federal FDA food code, and most jurisdictions have a process for variances to that code; there’s already a way for businesses to opt out, via variance, if they feel overburdened by the law as long as the outcome is the same.

Stuff like wild-grown mushrooms, ramps and game carry different risks because they aren’t in a managed system or environment. Misidentify a mushroom and a customer can die. Hunting morels are big business and many of the foraged fungi end up in restaurants sold on somewhat of a black market.

One way to encourage [better risk management] is to build more collaborative relationships between chefs and inspectors, says Bridget Sweet, executive director of food safety at Johnson & Wales. “So many people hate the health department and don’t even know why,” she says. She’s heard the horror stories about people operating in secret and hoping they don’t get caught. She finds them immensely distressing. “It’s such a risk. Inspectors don’t want to shut businesses down. If they have a really good discussion, it will remove the barriers. The answer’s not an inherent no, it’s ‘How can you do this safely within the food code?’ ”

Does Europe have an egg problem? Salmonella cases no longer falling in the EU

The declining trend of salmonellosis cases in the EU has levelled off according to the annual report on zoonotic diseases published today.

The European Centre for Disease Prevention and Control reports that cases of Salmonella Enteritiis acquired in the EU have increased in humans by 3% since 2014. In laying hens, the prevalence increased from 0.7% to 1.21% over the same period. 

“The increase shown by our surveillance data is worrying and a reminder that we have to stay vigilant,” said Mike Catchpole, ECDC’s Chief Scientist. “Even in a state of high awareness and with national control programmes for S. Enteritidis in place, there is a need for continuing risk management actions at the Member State and EU level,” he added. 

Marta Hugas, EFSA’s Chief Scientist, said: “The decrease of Salmonella has been a success story in the EU food safety system in the last 10 years. Recent S. Enteritidis outbreaks contributed to a change in this trend in humans and poultry. Further investigations by competent authorities in the field of public health and food safety will be crucial to understand the reasons behind the increase.” 

There were 94 530 human cases of salmonellosis reported in the EU in 2016. S. Enteritidis – the most widespread type of Salmonella, accounted for 59% of all salmonellosis cases originating in the EU and is mostly associated with the consumption of eggs, egg products and poultry meat. 

Campylobacter and Listeria

Campylobacter, the most reported food-borne pathogen in humans, was detected in 246 307 people, an increase of 6.1% compared with 2015. Despite the high number of cases, fatalities were low (0.03%). Levels of Campylobacter are high in chicken meat.

Listeria infections, which are generally more severe, led to hospitalisation in 97% of reported cases. In 2016, listeriosis continued to rise, with 2 536 cases (a 9.3% increase) and 247 deaths reported. Most deaths occur in people aged over 64 (fatality rate of 18.9%). People over 84 are particularly at risk (fatality rate of 26.1%). Listeria seldom exceeded legal safety limits in ready-to-eat foods.

Salmonella food-borne outbreaks increasing 

The 4 786 food-borne disease outbreaks reported in 2016 represent a slight increase in comparison with 2015 (4 362 outbreaks), but the figure is similar to the average number of outbreaks in the EU during 2010–2016. 

Outbreaks due to Salmonella are on the rise, with S. Enteritidis causing one in six food-borne disease outbreaks in 2016. Salmonella bacteria were the most common cause of food-borne outbreaks (22.3%), an increase of 11.5% compared to 2015. They caused the highest burden in terms of numbers of hospitalisations (1,766; 45.6% of all hospitalised cases) and of deaths (10; 50% of all deaths among outbreak cases).

Salmonella in eggs caused the highest number of outbreak cases (1 882).

21 sick with E. coli O157 from Romaine lettuce in Canada: What says LGMA?

The Sponge-Bob leafy greens cone of silence has once again been deployed with 21 sick from E. coli O157 linked to Romaine lettuce.

The Public Health Agency of Canada made the announcement Monday night, so it’s time for another edition of spokesthingy fairytales. The statements in italics are from the Public Health Agency of Canada. The comments below are mine.

The Public Health Agency of Canada is collaborating with provincial public health partners, the Canadian Food Inspection Agency and Health Canada.

How do you know a Canadian is making a presentation at a scientific conference? They spend half the talk noting collaborations.

At this time, there are no product recalls associated with this outbreak. The outbreak investigation is ongoing, and this public health notice will be updated on a regular basis as the investigation evolves.

We’ll see.

The risk to Canadians is low.

If you have no product recalls, no real clues other than some lettuce, how can you say the risk is low?

However, Canadians are reminded to follow safe food handling practices for lettuce to avoid becoming ill.

Washing does almost nothing to improve the safety of leafy greens, other than to remove the snot from an appropriately heightened 5-year-old and make consumers feel better.

Most people with an E. coli infection will become ill for a few days and then recover fully.

Unless you’re part of the percentage that has life-long health issues from eating a salad.

Some E. coli infections can be life threatening, though this is rare.

But it really sucks when it happens.

Currently, there are 21 cases of E. coli O157 illness under investigation in three provinces: Quebec (3), New Brunswick (5), and Newfoundland and Labrador (13).

These are not prime lettuce growing areas in Canada, in November.

Individuals became sick in November 2017. Ten individuals have been hospitalized.

No deaths have been reported.

See, no biggie. But this was issued Dec. 11. 2017. So was it early November or late November. The difference is hugely significant when assessing the timeliness of the announcement.

The Canadian Food Inspection Agency is working with public health officials to determine the source of the romaine lettuce that ill individuals were exposed to.
Which will be buried and vaguely released in some journal article a couple of years from now.
The following food safety tips for lettuce will help you reduce your risk of getting an E. coli infection.

  • Wash your hands thoroughly with warm water and soap for at least 20 seconds, before and after handling lettuce.

Water temperature doesn’t matter.

  • Discard outer leaves of fresh lettuce.
  • Wash your unpackaged lettuce under fresh, cool running water. There is no need to use anything other than water to wash lettuce. Washing it gently with water is as effective as using produce cleansers.

And neither are effective at removing pathogens.

  • Keep rinsing your lettuce until all of the dirt has been washed away.
  • Don’t soak lettuce in a sink full of water. It can become contaminated by bacteria in the sink.
  • Ready-to-eat lettuce products sold in sealed packages and labelled as washed, pre-washed or triple washed do not need to be washed again.
  • Use warm water and soap to thoroughly wash all utensils, countertops and cutting boards before and after handling lettuce to avoid cross-contamination.
  • Store lettuce in the refrigerator for up to seven days. Discard when leaves become wilted or brown.
  • Bagged, ready-to-eat, pre-washed lettuce products should also be refrigerated and used before the expiration date.

Sounds like Walkerton: More than 750,000 in NZ exposed to potentially unsafe drinking water

Tracy Watkins of Stuff writes complacency, inept officials – a Government inquiry paints a frightening picture of the state of New Zealand’s drinking water, with at least 750,000 of New Zealanders drinking from supplies that are “not demonstrably safe” – a figure described as likely to be a “significant underestimate.”

The inquiry was sparked by the 2016 Havelock North gastro outbreak, which has now been linked to four deaths, and calls for a major overhaul of water supplies, including mandatory treatment.

The Government has now written urgently to all mayors and district health boards asking to check the water they are supplying meets current standards after the inquiry revealed 20 per cent of water supplies were not up to standard.

That 20 per cent affects 759,000 people, of which 92,000 are at risk of bacterial infection, 681,000 of protozoal infection and 59,000 at risk from the long term effects of exposure to chemicals through their water supply.

But that figure was likely to understate the problem, as it did not include more than 600,000 people who drink water from self-suppliers or temporary suppliers, or tourists to places like Punakaiki on the West Coast, which is under a permanent “boil water” notice.

The inquiry found that complacency about the state of New Zealand’s drinking water was common, yet the evidence showed that in many cases it was safer to drink tap water overseas than here.

But its most damning findings related to the Ministry of Health, which it described as inept and negligent in its oversight of a system in which non-compliance with safe standards was high.

The risks for contamination of the water supplies were detailed by the inquiry including damaged pipes, a huge number of private and unknown bores, and the close proximity of sewerage to drinking water assets, a factor that caused surprise among overseas experts.

The second part of the inquiry looked at broader water quality issues.

It found that lessons from Havelock North appeared not to have been learned – compliance figures in the 2016-17 period were still “alarmingly low” and “do not appear to reflect any increased vigilance by suppliers in the aftermath of [that] outbreak”.

“The inquiry found the falling compliance levels with the bacteriological and chemical standards particularly concerning. The decrease in compliance with the bacteriological standards results from an increased number of transgressions, an increased number of supplies with ineffective, delayed or unknown remedial action following transgressions, and an increased number of supplies with inadequate monitoring.

“Twenty-seven supplies failed entirely to take any remedial action after a transgression. In the aftermath of the bacteriological outbreak in Havelock North, these failures to respond effectively to transgressions or to monitor adequately are surprising and unacceptable.”

Steam with your melons? If it makes produce safer, why not

Abstract

The purpose of this study was evaluation of the effectiveness of superheated steam (SHS) on inactivation of foodborne pathogens on cantaloupes and watermelons.

Saturated steam (SS) treatment was performed at 100 °C and that of SHS at 150 and 200 °C. Escherichia coli O157:H7, Salmonella Typhimurium and Listeria monocytogenes-inoculated cantaloupes and watermelons were exposed for a maximum of 30 s and 10 s, respectively. Populations of the three pathogens on cantaloupes and watermelons were reduced by more than 5 log after 200 °C steam treatment for 30 s and 10 s, respectively. After SHS treatment of cantaloupes and watermelons for each maximum treatment time, color and maximum load values were not significantly different from those of untreated controls. By using a noncontact 3D surface profiler, we found that surface characteristics, especially surface roughness, is the main reason for differences in microbial inactivation between cantaloupes and watermelons. The results of this study suggest that SHS treatment can be used as an antimicrobial intervention for cantaloupes and watermelons without inducing quality deterioration.

NC State University hosts a norovirus…outbreak

The following message popped into my email inbox earlier today:

Since Tuesday, Dec. 5, several students have reported experiencing gastrointestinal illness. Late yesterday, the Wake County Human Services Department confirmed the cause as norovirus.

Norovirus is a very contagious virus that causes stomach pain, nausea and diarrhea. More information about norovirus, and tips to prevent it from spreading, are available from the Centers for Disease Control and Prevention (CDC).

At present, approximately 60 students have exhibited norovirus-like symptoms. Most of the affected students live in Alexander Hall, however, additional cases of ill students have been received from a handful of other on- and off-campus housing locations.

With norovirus and other gastrointestinal illnesses, the most effective way to stop the spread is to practice good handwashing and personal hygiene.

If you are exhibiting symptoms and feel ill, you should thoroughly wash your hands after any bathroom visit. If you are feeling ill, you should not prepare food for or serve food to others. It is also important to get adequate rest and good oral hydration, both when ill and when trying not to become ill.

The university is taking every precaution to contain the spread of the illness, and to assist ill students, including the following actions:

• Student Health is actively working with University Housing to contact all identified sick students who live on campus to check on their health and needs.
• Wellness kits containing liquids and easy-to-digest foods have been provided to affected students.
• Students exhibiting gastrointestinal issues have been instructed to remain in their residences throughout their illness in an effort to not spread the virus to others.
• University Housekeeping staff have increased cleaning operations in affected areas as a precaution, including cleaning restrooms, hand railings, door knobs, etc., and will continue to do so daily until the illness passes.
• University Housekeeping will provide approved cleaning supplies to affected students for their university-owned personal living spaces.
• Faculty of the students who are ill have been notified.

Any students who are presenting symptoms should remain in their rooms, and on-campus students should contact their RA. Students experiencing persistent, severe vomiting or diarrhea should go to the Student Health Center, personal health care provider, or emergency healthcare facility. Students who are not sick should go about their normal routines.

Fonterra fined $183m over contamination scandal

New Zealand’s Fonterra has been ordered to pay 105 million euros (NZ$183 million) in damages to French food giant Danone as a result of the Fonterra food safety failures of 2013.

Danone had sued Fonterra as a result of the whey protein concentrate contamination scandal in 2013, when Fonterra quarantined several batches over fears it was contaminated with clostridium bacteria. It later turned out to be a false alarm.

Danone launched a legal suit in New Zealand and arbitration proceedings in Singapore, seeking restoration for the costs of recalling the whey protein concentrate.

At the time, Fonterra said it expected any court action would show the Kiwi firm didn’t have any liability in the contract, and it recognised a contingent liability of just $14m over the recall.

In 2014, New Zealand’s Court of Appeal upheld an earlier decision that the Singapore arbitration proceedings should be the first avenue, as provided for in the contract, but refused to permanently stay the legal suit.

The result of the Singapore proceedings was released on Friday, and Danone says it “welcomes” the decision.

36 dead: FDA says beware the herb kratom

The U.S. Food and Drug Administration has issued a strong warning to consumers to stay away from the herbal supplement kratom, saying regulators are aware of 36 deaths linked to products containing the substance.

Laurie McGinley of The Washington Post writes consumers are increasingly using the supplement, which comes from a plant in Southeast Asia, for pain, anxiety and depression, as well as symptoms of opioid withdrawal. The herb also is used recreationally because it produces symptoms such as euphoria.  Proponents say it is a safe way to deal with chronic pain and other ailments, and some researchers are exploring its therapeutic potential, including helping people overcome addictions.

But in a statement, FDA Commissioner Scott Gottlieb said that there is no “reliable evidence” to support the use of kratom as a treatment for opioid-use disorder, and that there are no other FDA-approved uses for kratom.

Rather, he said, evidence shows that the herb has similar effects to narcotics like opioids, “and carries similar risks of abuse, addiction and, in some cases, death.” He said that calls to U.S. poison control centers involving kratom increased tenfold between 2010 and 2015, and that the herb is associated with side effects including seizures, liver damage and withdrawal symptoms.

My daughter calls me flyslayer: I can’t stand flies, but are they a food service risk?

For decades, various food safety-types have debated the role of flies in the transmission of disease in food service settings, and more importantly, the significance.

Now that we live in an old distinguished house in Brisbane, and in every other place we’ve lived close to downtown (CBD) we know screens are an afterthought.

And maybe I’m an OK cook, because every time I start slicing and dicing, the flies converge, if the windows are open.

So we got central air installed – Brisbane breezes be damned – and keep the windows closed, yet I still go about my daily slaughter of about 50 flies.

Researchers at Penn State Eberly College of Science have found house flies carry salmonella, E. coli and even bacteria, which can lead to stomach ulcers and fatal sepsis.

The research, published in Scientific Reports, says flies may have been overlooked by public health officials as a source of disease outbreaks.

The paper found that flies legs transferred most of the microbial organisms from one surface to another, suggesting even a brief step onto food could leave behind bacteria.

Flies in urban areas were found to carry more bacteria than rural flies, with the scientists suggesting to avoid city parks for picnics and, instead, eating food in more rural locations.

The study suggests flies pick up the bacteria from faeces and decaying organic matter, which they use to nurture their young.

”People had some notion that there were pathogens that were carried by flies, but had no idea of the extent to which this is true and the extent to which they are transferred.” said Dr Donald Bryant, Professor of Biotechnology at Penn State University.

“We believe that this may show a mechanism for pathogen transmission that has been overlooked by public health officials.”
