Food safety training is like psychotherapy: Sure, I understand the theory, the neural pathways, the addictive brain, but will that change my behavior (shurley not).

But there’s always hope – in place of well-designed studies that measure success, failure, and actual experiments with novel approaches. Most studies get tossed on the rhetorical pile of we-need-more-education crap.

Improper food handling among those working in retail and food service settings is a frequent contributor to foodborne illness outbreaks. Food safety training and education interventions are important strategies to improve the behaviors and behavioral precursors (e.g., knowledge and attitudes) of food handlers in these settings.

We conducted a comprehensive systematic review to identify, characterize, and synthesize global studies in this area to determine the overall effectiveness of these interventions. The review focused on experimental studies with an independent control group. Review methods included structured search strategy, relevance screening of identified abstracts, characterization of relevant articles, risk of bias assessment, data extraction, meta-analysis of intervention effectiveness for four outcome categories (attitudes, knowledge, behavior, and food premise inspection scores), and a quality of evidence assessment.

We identified 18 relevant randomized controlled trials (RCTs) and 29 nonrandomized trials. Among RCTs, 25 (64%) unique outcomes were rated as high risk of bias, primarily owing to concerns about outcome measurement methods, while 45 (98%) nonrandomized trial outcomes were rated as serious risk of bias, primarily because of concerns about confounding bias. High confidence was identified for the effect of training and education interventions to improve food handler knowledge outcomes in eight RCT studies (standardized mean difference = 0.92; 95% confidence interval: 0.03, 1.81; I2 = 86%). For all other outcomes, no significant effect was identified. In contrast, nonrandomized trials identified a statistically significant positive intervention effect for all outcome types, but confidence in these findings was very low due to possible confounding and other biases.

Results indicate that food safety training and education interventions are effective to improve food handler knowledge, but more evidence is needed on strategies to improve behavior change.

Gaps and common misconceptions in public’s food safety knowledge

Background: Incidence rates of some foodborne illnesses (FBIs) in BC still remain on the rise despite numerous initiatives to prevent FBIs. This rise over the years has been attributed to gaps in the public’s food-safety knowledge and practices. In order to decrease incidence rates and prevent future FBIs, efforts should be made to identify common misconceptions in the public’s food safety knowledge. With a focus on the Metro Vancouver population, common misconceptions in food safety were found and their knowledge level towards the misconceptions was analyzed.

Methods: An in-person survey was conducted in three locations in Metro Vancouver. The survey asked for demographics information, perceived food safety knowledge and food safety misconceptions. ANOVA and Independent Sample T-test were administered to analyze results.

Results: No statistically significant difference in food safety knowledge was found between groups by gender, age, and geographic region. The majority of participants rated their food safety knowledge as moderate but they demonstrated a poor knowledge level in food safety.

Conclusion: The public’s knowledge level should be improved to prevent further rises of FBIs. Initiatives involving the provincial Foodsafe certification program, secondary school curriculums and health authority websites can be utilized to educate the public.

Most sushi producers in Ireland fail to meet food safety standards

I don’t like sushi and don’t eat it, especially the real kind with raw fish.

Andrew Lowth of Spin 1038 reports that checks have found almost every sushi maker and seller in Ireland has failed to meet food safety standards.

Of eleven manufacturers, restaurants and takeaways inspected, just one hadn’t broken food hygiene laws.

The Food Safety Authority found 76 breaches of food law, including poor parasite control and incorrect defrosting of raw fish.

The audit findings also include:

90% of food businesses audited did not have adequate controls in place relating to sushi production and processing activities

75% of food businesses did not meet the requirements of the legislation for  freezing fish for parasite control

Over 90% of the food businesses did not have adequate operational controls for sushi rice production

“Our audit sought to establish if food safety controls were being followed and the findings are very concerning,” says FSAI CEO Pamela Byrne.

“It showed that over three quarters of the food businesses did not have adequate food safety controls in place for this.”

84 raspberry pickers stricken with leptospirosis in Australia, 2018

In 2018, an outbreak of leptospirosis was identified among raspberry workers from a mixed‐berry farm in New South Wales, Australia. Initial testing had not revealed a cause, but eventually leptospirosis was detected via polymerase chain reaction (PCR). Further serological testing detected Leptospira borgpetersenii serovar Arborea, of which rodents are the predominant reservoir. Leptospirosis is rare in Australia, with outbreaks usually related to flooding. We conducted an investigation to identify risk factors for infection, to inform control measures.

Cases were detected through laboratory notifications, hospital‐based syndromic surveillance, awareness‐raising among farm employees and clinician alerts. Confirmed cases had a four‐fold rise in antibody titre or single titre ≥400 on microscopic agglutination test, and a positive IgM. Probable cases had a positive Leptospira PCR or IgM, and possible cases had a clinically compatible illness. We conducted a case–control study among raspberry workers on the farm and compared reported exposures between cases and seronegative controls. We assessed environmental risks on‐site and tested rodents for leptospirosis.

We identified 84 cases over a 5‐month period (50 confirmed, 19 probable and 15 possible). Compared with controls, cases were less likely to wear gloves and more recently employed. Cases also more commonly reported always having scratched hands, likely from the thorns on raspberry plants. We observed evidence of rodent activity around raspberry plants and three of thirteen trapped mice tested positive for Leptospira Arborea. Control measures included enhanced glove use, doxycycline prophylaxis and rodent control.

This is the largest known outbreak of leptospirosis in Australia. Workers were likely exposed through scratches inflicted during harvesting, which became contaminated with environmental leptospires from mice. Leptospirosis should be considered an occupational risk for raspberry workers, requiring protective measures. Chemoprophylaxis may assist in controlling outbreaks. PCR assists in the early diagnosis and detection of leptospirosis and should be included in surveillance case definitions.

UK factory put uncooked sausage in pre-packed sandwiches posing ‘danger to health’

James Cain of The Mirror reports a factory has been ordered to stop making food after it put uncooked sausages into pre-packed sandwiches.

The Middlesbrough-based factory has been told by health authorities it risked causing a listeria outbreak.

Café Class Ltd has been served with a hygiene emergency prohibition order for food safety practices that posed an “immediate danger to human health”.

A court this week heard how the company extended the use-by dates of boiled eggs, cheddar cheese and streaky bacon, putting consumers’ health at risk.

The risk to the public was so severe that the Food Standards Agency (FSA) issued an immediate product recall on sandwiches, wraps and salads made by the company.

Listeria has been in the spotlight this year after six people died after getting listeria from prepackaged sandwiches and salads served in UK hospitals.

In the unrelated case the company, which traded with stores including Londis, Nisa and North East Convenience Stores, faced court as Middlesbrough Council sought an emergency hygiene order to prevent it from making food, reports the Local Demoracy Reporting service for Teesside Live .

Company directors Shahid Nawaz and Mohammed Haris Abdullah arrived at Teesside Magistrates’ Court yesterday to hear Middlesbrough Council lay out the case against their company.

Andrew Perriman, prosecuting for the council , told magistrates that Café Class, based in Riverside Park, was visited by environmental health officers on September 9.

The inspection was arranged “to assess compliance with a hygiene improvement notice served earlier in the year as a result of allergy management concerns”.

But Mr Perriman said the officers were shocked to discover the factory was routinely placing ingredients on their use-by date in sandwiches, wraps and salads which would then be labelled with a four-day use-by date.

“In respect of cooked ham aspect used in the final product, it is specified by the manufacturer to be used within three days once opened,” said Mr Perriman.

But the officers found that once opened, the ham had been placed in a plastic container on September 8 and labelled with a use-by date of September 11.

Mr Perriman said it could be argued that if September 8 is counted as day one, this actually meant the ham was being used for four days.

In any case, the factory would continue to use the ham as an ingredient right up until the final use-by day.

But Mr Perriman added: “It was then placed into a sandwich and given a further four-day use by date.

“Not only that, the packaging on the final product stated ‘once opened consume within 24 hours’.

He said this practice meant cooked ham with a use-by date of September 10 or 11 was actually being used in a product labelled with a use-by date of September 15 or 16.

“As a result, the three-day shelf life is exceeded by a further six days,” he said adding that this was “way past” safe limits.The company’s website says: “We here at Café Class carefully ensure that the standards of Food Agency are met at all times and any waste is disposed of appropriately.

“All our products are fully cooked but we do not send the food waste to landfill sites, thus helping the environment and fulfilling our responsibility towards the society.”

Uh huh.

Yes Minister: Dozens of people stricken by Salmonella-infected British eggs

Dozens of people have been poisoned after consuming British eggs contaminated with salmonella, an investigation has found, despite recent government assurances that the risk had been virtually eliminated.

Andrew Wasley of The Guardian reports at least 45 consumers have fallen ill since January this year in a major disease outbreak health officials have traced back to contaminated eggs and poultry farms.

Despite outbreaks of this strain occurring for more than three years, the government has issued no public warnings about the safety of hens’ eggs. In 2017, the Food Standards Agency (FSA) told the public that it was safe for vulnerable people, including pregnant women and the elderly to eat raw, runny or soft-boiled eggs. At the time the head of the FSA said: “The risk of salmonella is now so low you needn’t worry.”

Internal records obtained by the Bureau of Investigative Journalism and the Guardian show that 25 egg-laying poultry flocks in the UK have tested positive for salmonella in 2019 so far, seven of them contaminated with the most serious strains of the bacteria. Two egg-packing factories – one that supplies leading supermarkets – have also been contaminated, records show.

Eggs produced by the infected poultry flocks were placed under restrictions, meaning they cannot be sold to the public and must be sent for processing to kill the bacteria or be disposed of – while birds from infected flocks were culled.

However, some contaminated eggs did reach the public, with PHE confirming that 45 people had become ill after eating eggs infected with salmonella since January. The exact route to the public is unclear.

The government records also reveal that in 2018, 28 flocks tested positive for salmonella, four of them with dangerous strains.

According to PHE a further 55 human cases prior to 2019 were also being linked to the outbreak.

The revelations come just two years after the Food Standards Agency (FSA) declared that almost all eggs produced in the UK were free of salmonella. A major health scare in the 1980s had led to warnings that vulnerable groups should not consume raw or lightly cooked eggs – or food containing them – because of the salmonella risk. The then junior health minister Edwina Currie sparked a public outcry after saying “most” British egg production was infected with salmonella.

But in 2017 the FSA lifted the advice, stating the presence of salmonella in eggs had been “dramatically reduced” and that “British Lion” eggs – which cover about 90% of UK egg production – were safe to eat.

Speaking at the time, the then FSA chair Heather Hancock said: “We are now saying if there is a British Lion egg, you’re safe to do that. The risk of salmonella is now so low you needn’t worry. And that’s true whether you’re a fit healthy adult, or whether you’re pregnant or elderly or young. It’s only people on strictly medically supervised diets who need to avoid those eggs.”

PHE stated that it had been investigating this strain of salmonella for three years, despite the FSA clearing eggs for consumption.

The British Retail Consortium said: “Food safety remains a top priority for UK retailers and all UK sourced eggs are produced to the Lion code of practice. Retailers will comprehensively investigate any safety issues in our food supply and will take swift action as necessary.”

Ethanol may not be so good: Better hand hygiene for flu prevention

The American Society for Microbiology says rubbing hands with ethanol-based sanitizers should provide a formidable defense against infection from flu viruses, which can thrive and spread in saliva and mucus. But findings published this week in mSphere challenge that notion — and suggest that there’s room for improvement in this approach to hand hygiene.

The influenza A virus (IAV) remains infectious in wet mucus from infected patients, even after being exposed to an ethanol-based disinfectant (EBD) for two full minutes, report researchers at Kyoto Profectural University of Medicine, in Japan. Fully deactivating the virus, they found, required nearly four minutes of exposure to the EBD.

The secret to the viral survival was the thick consistency of sputum, the researchers found. The substance’s thick hydrogel structure kept the ethanol from reaching and deactivating the IAV. 

“The physical properties of mucus protect the virus from inactivation,” said physician and molecular gastroenterologist Ryohei Hirose, Ph.D, MD., who led the study with Takaaki Nakaya, PhD, an infectious disease researcher at the same school. “Until the mucus has completely dried, infectious IAV can remain on the hands and fingers, even after appropriate antiseptic hand rubbing.

The study suggests that a splash of hand sanitizer, quickly applied, isn’t sufficient to stop IAV. Health care providers should be particularly cautious: If they don’t adequately inactivate the virus between patients, they could enable its spread, Hirose said. 

The researchers first studied the physical properties of mucus and found — as they predicted — that ethanol spreads more slowly through the viscous substance than it does through saline. Then, in a clinical component, they analyzed sputum that had been collected from IAV-infected patients and dabbed on human fingers. (The goal, said Hirose, was to simulate situations in which medical staff could transmit the virus.) After two minutes of exposure to EBD, the IAV virus remained active in the mucus on the fingertips. By four minutes, however, the virus had been deactivated.

Previous studies have suggested that ethanol-based disinfectants, or EBDs, are effective against IAV. The new work challenges those conclusions. Hirose suspects he knows why: Most studies on EBDs test the disinfectants on mucus that has already dried. When he and his colleagues repeated their experiments using fully dried mucus, they found that hand rubbing inactivated the virus within 30 seconds. In addition, the fingertip test used by Hirose and his colleagues may not exactly replicate the effects of hand rubbing, which through convection might be more effective at spreading the EBD.
 For flu prevention, both the Centers for Disease Control and Prevention and the World Health Organization recommend hand hygiene practices that include using EBDs for 15-30 seconds. That’s not enough rubbing to prevent IAV transmission, said Hirose. 
 The study wasn’t all bad news: The researchers did identify a hand hygiene strategy that works, also sanctioned by the WHO and CDC. It’s simple: Wash hands, don’t just rub them. Washing hands with an antiseptic soap, they found, deactivated the virus within 30 seconds, regardless of whether the mucus remained wet or had dried. 

How a frozen meringue led Australian investigators to the source of a potent Salmonella outbreak

Jess Davis of ABC News reports a frozen meringue was key to identifying and outbreak of Salmonella enteritidis (SE), a bacteria that until last year was not found in Australia, that sickened almost 200 people.

People first started getting sick in May 2018 and by July a cluster of cases had appeared in New South Wales. That was when health authorities started investigating.

“Health, through their investigations, were able to look at a number of isolates of Salmonella enteritidis that came from humans, who unfortunately had been ill, and use a technology called whole genome sequencing,” said NSW Food Authority CEO Lisa Szabo.

“So it’s a genetic-based technology that helps us join the dots, shall I say. And this was the first time they could see a group of people with the same whole genome sequence.”

Anyone with a confirmed case of SE was interviewed by investigators and asked for a detailed account of what they’d eaten — to try to find what the different cases had in common.

A few weeks after being interviewed, one of those people remembered they had a frozen meringue cake in their freezer, leftover from a birthday party, around the time they got sick.

Officers went to that person’s home, collected the cake and had it tested.

“We were able to isolate the Salmonella enteritidis and it had that same whole genome sequence. At the same time we could see who manufactured that cake,” Ms Szabo said.

“We could go back to the manufacturer, have a look at their environment, look at how they handle food and where they get their ingredients from, and that’s where we saw the connection to the egg farm.”

It wasn’t until September that the frozen meringue led investigators to a farm on the outskirts of Sydney, but by then the bacteria had slowly started spreading across the industry.

“Once we detected salmonella enteritidis on this particular farm, we then commenced another round of investigations … more from the biosecurity and then the farm side of trying to understand … [whether the] farm had other connections to other properties around the state” Ms Szabo said.

But how the bacteria made its way into Australian eggs in the first place is likely to remain a mystery.

One property in Victoria and 13 in NSW have been affected so far and more than half-a-million birds have been culled at a cost of $10 million.

The spread of SE has been blamed largely on the interconnected nature of the egg industry, with all the infected farms connected in some way.

Egg farmers often trade produce with each other, and equipment and workers also regularly move from farm to farm.

Veterinarian Rod Jenner said SE was difficult to contain because it could survive and multiply without a host and could live in the environment for up to two years.

“It can survive in dust and dirt, in vehicles, and can travel in the wind. Rodents, wild birds, that sort of thing, can carry it on their skin or in their bodies as well,” he said.

“So it has actually been demonstrated to travel vast distances and be contaminated, be deposited on other farms that have previously been free.”

A farmer’s worst nightmareBede Burke’s egg farm at Tamworth in NSW was the 11th property to be infected, with a notification it had tested positive to SE during a routine check just over three months ago.

“Your whole world crashes down around you, you know,” Mr Burke said.

“We just didn’t sleep for a week and that first seven or eight days was really traumatic. We had to learn how to both decontaminate and disinfect the premises.”

When the notification came through on the eve of the federal election, Mr Burke had to withhold his eggs from sale and was faced with the prospect of culling entire flocks.

“But then you’ve got heap of eggs on your premises, you can’t not stop packing eggs, we were still going to pack 90,000 eggs a day,” he said.

“It’s just stress beyond all belief and then start planning for the worst.”

But he was lucky the contamination was picked up early and while a swab of dirt and dust had tested positive, it hadn’t yet spread to his egg or birds.

There have been no confirmed cases of SE since June and the industry hopes that will be the end of it.

But the outbreak has raised serious questions about how biosecurity is managed. Despite the disease becoming a national problem, its enforcement and regulation is state-based.

Philip Szepe, who runs an egg farm at Kinglake in Victoria, tests for all strains of salmonella every three months.

But he’s concerned that not all farmers are as diligent and said biosecurity was too reliant on self-regulation.

“Government’s really good at responding to crisis. It’d be great if the Government had a bit more engagement with the industry around monitoring, surveillance and compliance,” he said.

Sweden: Mouse pies on fallow sausage in school kitchen—high levels of E. coli

One morning when today’s school lunch was to be cooked at the Torskolan in Torsås, the staff discovered that the falu sausage that would have been cooked was covered with musbajs.

The school called me and told me that something similar to rodent spilling was found on the fallow basket. We then contacted the company from which we buy the sausage, which immediately withdrew it, says Gustaf Nilsson, environmental inspector at Torsås Municipality.

The planned sausage lunch was quickly replaced with soup and a detective work has been underway to find out where the mice have come in.

“I don’t know where they managed to get in, but before it came to school, the sausage was both at a factory and at a transhipment center. However, it is unlikely that it happened at school. The staff has detailed their routines there.

Gustaf Nilsson says he has not been to anything like it before.

“After all, the routines have failed and it is very unfortunate that this has happened. However, the staff at Torskolan did everything right to discover the pellet before starting to cook.”

Samples taken on the sausage show high levels of E. coli, ie some form of stool. The suspicion is that it comes from forest mice.

CFIA: Establishment-based risk assessment model for food establishments

Why do government-types never learn to write in clear and coherent sentences?

CFIA has been evolving the way we manage risk, to further support industry’s ability to compete globally, and embracing technology to provide more efficient and responsive services.

The Establishment-based Risk Assessment (ERA) model for food establishments is a tool developed by the CFIA to evaluate food establishments based on the level of risk they represent to Canadian consumers. The ERA model uses data and a mathematical algorithm to assess the food safety risks of food establishments under CFIA jurisdiction. It takes into consideration risks associated with a specific food commodity, operation or manufacturing process, mitigation strategies implemented by the industry to control their food safety risks, as well as establishment compliance information. The ERA model will be used, along with other factors, to inform where inspectors should spend more or less time and inform program planning, in order to focus efforts on areas of highest risk.

How does the ERA model for food establishments work?

The ERA model has already garnered attention on the international stage. Indeed, scientific journals dedicated to publishing the latest research on food safety and risk analysis such as Microbial Risk AnalysisFood MicrobiologyFood Control and International Journal of Food Microbiology have published articles detailing the development steps of CFIA’s ERA model.

Yeah, and I get quoted every day, but make nothing, whereas government-types have their salary and pension and endless meetings.

Surveys still suck: Did the UK FSA discover that piping hot is a fairytale

The Food Standards Agency (FSA) have released their biannual findings from the general public attitudes tracker. This tracker highlights the behaviour, thoughts and reputation of food safety aspects throughout the year. Whenever there’s a scandal, a legislation change or a news piece surrounding the FSA’s points of interest, it’s going to have a public reaction. Whether good or bad, these reactions will shape and alter the way in which the public perceives food safety.

The FSA’s findings are based on 2,150 interviews from a representative sample of adults aged 16 and over across England, Wales and Northern Ireland. Fieldwork was carried out between 8th and 26th May 2019, as part of the regular TNS Kantar face-to-face-omnibus survey.

Questions cover several topics of interest for the Agency, including:

concern about food safety issues

awareness of food hygiene standards

awareness of the FSA and its responsibilities

trust in the FSA and the food industry

confidence in food labelling.

At wave 18, a new set of questions were added to monitor the public’s trust in the FSA as well as the wider food system.

One of the FSA’s strategic objectives is to ensure consumers have the information and understanding to make informed choices about where and what they eat. To help monitor performance against this objective, respondents were asked about their awareness of hygiene standards when buying food or eating out. At wave 18, 52% of respondents reported always being aware of the hygiene standards in places they eat out at or buy food from, and a further 33% said they were sometimes aware.

It’s fair to say that the public is now taking a greater interest in UK food safety standards, meaning there is less margin for error in the food industry. With nearly 80% of the UK public being aware of food hygiene standards when eating out, it’s imperative that you get your standards right first time.
