Multistate outbreak of Shiga toxin-producing E. coli O157:H7 infections (STEC O157:H7) in 13 states

Eastern Canada has been experiencing an outbreak of E. coli O157:H7 infections linked to romaine lettuce. Since the lettuce is eaten raw, this increases the likelihood of acquiring the infection. It appears now that the states are experiencing a similar outbreak. The CDC is performing whole genome sequencing to determine if this outbreak is related to the Canadian romaine lettuce outbreak.

CDC, several states, and the U.S. Food and Drug Administration are
investigating a multistate outbreak of Shiga toxin-producing E. coli O157:H7 infections (STEC O157:H7) in 13 states. Seventeen illnesses have been reported from California (3), Connecticut (2), Illinois (1), Indiana (1), Michigan (1), Nebraska (1), New Hampshire (2), New York (1), Ohio (1), Pennsylvania (1), Virginia (1), Vermont (1) and Washington (1). Illnesses started on dates from November 15 through December 8, 2017. The Public Health Agency of Canada also is investigating an outbreak of STEC O157:H7 infections in several provinces.
CDC is performing whole genome sequencing on samples of bacteria making people sick in the United States to give us information about whether these illnesses are related to the illnesses in Canada. Preliminary results show that the type of E. coli making people sick in both countries is closely related genetically, meaning the ill people are more likely to share a common source of infection.
The Public Health Agency of Canada has identified romaine lettuce as the source of the outbreak in Canada. In the United States, state and local public health officials are interviewing sick people to determine what they ate in the week before their illness started. CDC is still collecting information to determine whether there is a food item in common among sick people, including leafy greens and romaine.
Because we have not identified a source of the infections, CDC is unable to recommend whether U.S. residents should avoid a particular food. This investigation is ongoing, and more information will be released as it becomes available.

12 stricken with E. coli O157 in UK burgers linked to Sainsbury’s

A supermarket’s own brand of beef burgers have been recalled in fears that customers might be struck down with the E.coli bug, just before Christmas.

Sainsbury’s issued an urgent product recall after customers apparently fell ill after eating the Aberdeen British Beef burgers.

Public Health England (PHE) has linked a dozen cases of E.coli to the range and Sainsbury’s have cleared the product from their shelves. All the people that fell ill have all recovered, PHE have said.

The bug has been linked to Sainsbury’s Taste the Difference ­Aberdeen Angus quarter-pounder burgers, 454g packets, which are sold in the frozen section.

The Food Standards Agency warned shoppers: “If you have bought any of the batches, do not eat them.”

Jazz is music in hell: Odwalla founder finds second life in Califia almond and plant-based beverages

Geoffrey Mohan of the LA Times writes that Greg Steltenpohl, 63, heads Califia Farms, an almond and plant-based beverage company he co-founded in 2010. With about $100 million in annual sales, the company is something of a redemption for the Stanford graduate, whose first lightning strike in the beverage business, Odwalla, started as a way to fund his avant-garde jazz band, and ended with a fatal food poisoning and recall that eventually left the company in the hands of Coca-Cola.

“Odwalla got started because I didn’t really have a plan. I was focused on music and just thought, ‘Hey, I can make some juice on the side, play music and all that.’”

The band’s eclectic mixtures of unpasteurized juice were far more popular than the band’s music and, by 1993, Steltenpohl and his partners took Odwalla public.

Accidental success met accidental fall in 1996, the year Odwalla hit its peak sales of $59 million. An E. coli outbreak traced to Odwalla’s raw apple juice sickened dozens and killed a child in Colorado. Federal criminal charges, fines, lawsuit settlements and a precipitous drop in sales left the company so short of cash it wound up controlled by new investors who eventually sold the brand to Coca-Cola.

Steltenpohl tried his hand at several other businesses before getting a call from Berne Evans, the head of Sun Pacific packing, who had helped pioneer easy-peeling mandarins — trademarked Cuties.

Steltenpohl blanches at the idea that he has some knack for catching food preference waves just as they crest — with Odwalla, then with almond milk, and now with a line of almond-based cold brew coffee drinks.

“It sounds like that,” he admitted with a laugh. “But you figure I’ve been doing it for 37 years. You could say I hit the waves, but there’s a lot of paddling in there.”

“It’s not always the important thing to be the first,” Steltenpohl said. “I think it’s more important to solve a number of other problems.… The way we talk about it is: something different, something better — that’s kind of the hurdle we have to pass internally. If we can’t answer to ourselves why is it different, why is it better, how does it move the bar higher, then why are we doing it?”

Too bad you didn’t apply that to juice business.

 In late Oct. 1996, an outbreak of Escherichia coli O157:H7 was traced to juice containing unpasteurized apple cider manufactured by Odwalla in the northwest U.S.Sixty-four people were sickened and a 16-month-old died from E. coli O157:H7. During subsequent grand jury testimony, it was revealed that while Odwalla had written contracts with suppliers to only provide apples picked from trees rather than drops – those that had fallen to the ground and would be more likely to be contaminated with feces, in this case, deer feces — the company never verified if suppliers were actually doing what they said they were doing. Earlier in 1996, Odwalla had sought to supply the U.S. Army with juice. An Aug. 6, 1996 letter from the Army to Odwalla stated, “we determined that your plant sanitation program does not adequately assure product wholesomeness for military consumers. This lack of assurance prevents approval of your establishment as a source of supply for the Armed Forces at this time.”

Kansans, why are you taking rattlesnake pills

I didn’t know rattlesnake pills were a thing.

They are, and they made someone in Kansas sick with salmonellosis.

According to CDC, rattlesnake pills are often marketed as remedies for various conditions, such as cancer and HIV infection. These pills contain dehydrated rattlesnake meat ground into a powder and put into pill form.

Epidemiologic and laboratory evidence indicate that one person in Kansas became sick after taking rattlesnake pills purchased in Mexico. The ill person reported taking rattlesnake pills in the week before getting sick. Advanced laboratory testing called whole genome sequencing showed that the Salmonella that made the person sick matched the Salmonellafound in rattlesnake pills from Mexico collected in an earlier, unrelated investigation.

Wash your bagged lettuce? I don’t. Here’s why.

The coverage of an outbreak usually follows a familiar story arc – people get sick; additional illnesses roll in as the investigation unfolds; media shares what people can do with the product to protect themselves and finally, sometimes a source is fingered.

In Canada’s lettuce-linked E. coli O157 outbreak, we’re in phase three – what can you do?

One challenge in answering that question is the Public Health Agency of Canada hasn’t released (or doesn’t know) exactly what types of romaine lettuce are linked – and whether it’s sold as a head, or chopped (like in a salad kit). The ‘what can you do?’ is influenced by that.

According to Global News, one of the risk management steps a home chef could employ is washing the lettuce. Even if its washed, bagged and ready-to-eat.

Jason Tetro, a visiting scientist at the University of Guelph in Ontario, explained that the bacteria is typically not found in produce such as lettuce. Instead, it’s in the water that’s used to irrigate produce on farms. For example, he said the water from a nearby river could be contaminated with manure from cows on the farm.

But Tetro explained that if lettuce is properly washed, it can minimize the risk of contamination.

“If you are buying lettuce, romaine or otherwise, make sure each leaf is given a good washing under warm to hot water,” he advised.

Using “friction,” or rubbing the leaves with hands, is one way of making sure any bacteria is washed off.

Does pre-washed lettuce need to be rinsed?

The shorter answer, according to Tetro, is yes.

“Let’s put it this way, did you see the person who washed it? No,” he said, explaining that consumers can never be too sure of how the produce was cleaned.

“It’s much better for your own safety that you do something yourself.”

I disagree when it comes to prewashed bagged lettuce from a regulated processor who’s managing food safety, because of what these folks wrote:

A panel of scientists with expertise in microbial safety of fresh produce was convened to review recent research and re-evaluate guidelines for foodservice and restaurant operators, regulatory agencies with oversight over food facilities, and consumers for handling prewashed bagged salads. The guidelines developed by the panel, together with materials reviewed by the panel to develop the guidelines, are presented. The background materials reviewed include published research and recent recommendations made by other authoritative sources. The panel concluded that leafy green salad in sealed bags labeled “washed” or “ready-to-eat” that are produced in a facility inspected by a regulatory authority and operated under cGMPs, does not need additional washing at the time of use unless specifically directed on the label. The panel also advised that additional washing of ready-to-eat green salads is not likely to enhance safety. The risk of cross contamination from food handlers and food contact surfaces used during washing may outweigh any safety benefit that further washing may confer.

 

South African grocers urged to test food for Listeria. Not sure that would do much for public health

According to MSN, as a result of the massive, unsourced, outbreak of listeriosis in South Africa, folks have been asking retailers to test their food before selling it.

Following the announcement of an outbreak of listeria in South Africa, there are now calls for supermarkets to test their food for the bacteria before it’s sold to customers.

AfriForum’s Marcus Pawson says restaurants should also take the necessary precautions.

“These big chain stores must do this themselves while we wait for government to come up with a plan to ensure that South Africans are safe.”

Maybe there’s something lost in the scientific translation, but as lots of folks have mentioned in the past, you can’t test your way to safe food. Good epidemiology and a matching active food surveillance program is what’s really needed. Test and hold, not sure what that will really accomplish, public health-wise.

Canadian STEC/lettuce outbreak leads to a death

Every day I eat food that doesn’t kill me. Or hasn’t yet.

I get kinda emotional when I read about a death linked to food.

While billions of servings of food every year don’t lead to tragedy, when it does I take notice.

I’ve met folks who continue to suffer the life-long impacts of foodborne pathogens; people who have watched their loved ones in pain in a hospital bed and ultimately them.

All from food.

According to an updated statement from the Public Health Agency of Canada, 30 people across Canada are dealing with E. coli O157 linked to romaine lettuce.  There’s not a lot of details though.

Currently, there are 30 cases of E. coli O157 illness under investigation in five provinces: Ontario, (6), Quebec (5), New Brunswick (5), Nova Scotia (1), and Newfoundland and Labrador (13). Individuals became sick in November and December 2017. Twelve individuals have been hospitalized. One individual has died. Individuals who became ill are between the ages of 4 and 80 years of age. The majority of cases (70%) are female.

Many individuals who became sick reported eating romaine lettuce before their illnesses occurred. The Canadian Food Inspection Agency is working with public health officials to determine the source of the romaine lettuce that ill individuals were exposed to.

No one should die as a result of food they ate.

California sisters fighting to recover from E. coli

The Public Health Agency of Canada may think Shiga-toxin producing E. coli is no biggie, but tell that to the Niles sisters of southern California, who were both hospitalized with hemolytic uremic syndrome (HUS).

ABC 30 reports that Mariska and Willow were active and healthy kids with no medical history. Their parents thought it was a terrible case of the flu. Originally their pediatrician thought it was norovirus. But after days of worsening symptoms, they were admitted to Valley Children’s Hospital with E. coli.

13-year-old Mariska Niles is finally starting to improve after 16 days in the hospital. She’s had more blood transfusions than she can count along with excruciating stomach pain and she was hallucinating.

The sisters were diagnosed with E. coli HUS or typical hemolytic uremic syndrome but the girls had unique cases.

Dr. Molly Dorfman said, “There’s was pretty atypical. Particularly the severity of Willow’s case was very very severe.”

This form of bacteria usually originates from contaminated food or water products. Pinpointing the exact source has been difficult. They haven’t traveled anywhere recently. The family hadn’t eaten out lately. It’s likely other family members also ate what the girls did but did not become violently ill. Even more puzzling, Mariska and Willow rarely eat the same things.

9-year-old Willow’s kidney’s still are not working. She has been debilitated by toxins from the infection, and at one point couldn’t wake up. Both sisters have had blood transfusion and dialysis.

21 sick with E. coli O157 from Romaine lettuce in Canada: What says LGMA?

The Sponge-Bob leafy greens cone of silence has once again been deployed with 21 sick from E. coli O157 linked to Romaine lettuce.

The Public Health Agency of Canada made the announcement Monday night, so it’s time for another edition of spokesthingy fairytales. The statements in italics are from the Public Health Agency of Canada. The comments below are mine.

The Public Health Agency of Canada is collaborating with provincial public health partners, the Canadian Food Inspection Agency and Health Canada.

How do you know a Canadian is making a presentation at a scientific conference? They spend half the talk noting collaborations.

At this time, there are no product recalls associated with this outbreak. The outbreak investigation is ongoing, and this public health notice will be updated on a regular basis as the investigation evolves.

We’ll see.

The risk to Canadians is low.

If you have no product recalls, no real clues other than some lettuce, how can you say the risk is low?

However, Canadians are reminded to follow safe food handling practices for lettuce to avoid becoming ill.

Washing does almost nothing to improve the safety of leafy greens, other than to remove the snot from an appropriately heightened 5-year-old and make consumers feel better.

Most people with an E. coli infection will become ill for a few days and then recover fully.

Unless you’re part of the percentage that has life-long health issues from eating a salad.

Some E. coli infections can be life threatening, though this is rare.

But it really sucks when it happens.

Currently, there are 21 cases of E. coli O157 illness under investigation in three provinces: Quebec (3), New Brunswick (5), and Newfoundland and Labrador (13).

These are not prime lettuce growing areas in Canada, in November.

Individuals became sick in November 2017. Ten individuals have been hospitalized.

No deaths have been reported.

See, no biggie. But this was issued Dec. 11. 2017. So was it early November or late November. The difference is hugely significant when assessing the timeliness of the announcement.

The Canadian Food Inspection Agency is working with public health officials to determine the source of the romaine lettuce that ill individuals were exposed to.
Which will be buried and vaguely released in some journal article a couple of years from now.
The following food safety tips for lettuce will help you reduce your risk of getting an E. coli infection.

  • Wash your hands thoroughly with warm water and soap for at least 20 seconds, before and after handling lettuce.

Water temperature doesn’t matter.

  • Discard outer leaves of fresh lettuce.
  • Wash your unpackaged lettuce under fresh, cool running water. There is no need to use anything other than water to wash lettuce. Washing it gently with water is as effective as using produce cleansers.

And neither are effective at removing pathogens.

  • Keep rinsing your lettuce until all of the dirt has been washed away.
  • Don’t soak lettuce in a sink full of water. It can become contaminated by bacteria in the sink.
  • Ready-to-eat lettuce products sold in sealed packages and labelled as washed, pre-washed or triple washed do not need to be washed again.
  • Use warm water and soap to thoroughly wash all utensils, countertops and cutting boards before and after handling lettuce to avoid cross-contamination.
  • Store lettuce in the refrigerator for up to seven days. Discard when leaves become wilted or brown.
  • Bagged, ready-to-eat, pre-washed lettuce products should also be refrigerated and used before the expiration date.
