Still a failure to communicate and, says CDC, sprouts still suck

After a series of outbreaks associated with sprouts in the mid-1990s, the U.S. Food and Drug Administration (FDA) published guidelines in 1999 for sprouts producers to reduce the risk of contamination. The recommendations included treating seeds with an antimicrobial agent such as calcium hypochlorite solution and testing spent irrigation water for pathogens.

jimmy.john's.sproutsFrom 1998 through 2010, 33 outbreaks from seed and bean sprouts were documented in the United States, affecting 1330 reported persons. Twenty-eight outbreaks were caused by Salmonella, four by Shiga toxin–producing Escherichia coli, and one by Listeria. In 15 of the 18 outbreaks with information available, growers had not followed key FDA guidelines. In three outbreaks, however, the implicated sprouts were produced by firms that appeared to have implemented key FDA guidelines.

Although seed chlorination, if consistently applied, reduces pathogen burden on sprouts, it does not eliminate the risk of human infection. Further seed and sprouts disinfection technologies, some recently developed, will be needed to enhance sprouts safety and reduce human disease. Improved seed production practices could also decrease pathogen burden but, because seeds are a globally distributed commodity, will require international cooperation.

Raw sprouts are the poster child for failures in what academics call, risk communication.

I’ve been thinking about this for a long time.

sprout.santa.barf.xmasAbout 1999, graduate student Sylvanus Thompson started working with me on risk analysis associated with sprouts. He got his degree and went on to rock-star status in the food safety world with the implementation of the red-yellow-green restaurant inspection disclosure program with Toronto Public Health, but we never published anything.

I remember frantically flying to Kansas City to hang out with this girl in Manhattan (Kansas) I’d met a couple of weeks before, in the midst of the 2005 Ontario raw sprout outbreak that sickened over 700; Jen Tryon, now with Global News, interviewed me at the airport, with me wearing a K-State hockey shirt (that’s the joke; there is no hockey at K-State, and I was still employed by Guelph; and I was going to hang out with this girl).

After the German E. coli O104 outbreak that killed 53 people last year and sickened over 4,000, along with the ridiculous public statements and blatant disregard for public safety taken by sandwich artist Jimmy John’s in the U.S., I figured we really needed to publish something.

The basic conclusions:

• raw sprouts are a well-documented source of foodborne illness;

• risk communication about raw sprouts has been inconsistent; and,

• continued outbreaks question effectiveness of risk management strategies and producer compliance

We document at least 55 sprout-associated outbreaks occurring worldwide affecting a total of 15,233 people since 1988. A comprehensive table of sprout-related outbreaks can be found at http://bites.ksu.edu/sprouts-associated-outbreaks.

Sprouts present a unique food safety challenge compared to other fresh produce, as the sprouting process provides optimal conditions for the growth and proliferation of pathogenic bacteria. The sprout industry, regulatory agencies, and the academic community have been collaborating to improve the microbiological safety of raw sprouts, including the implementation of Good Manufacturing Practices (GMP), establishing guidelines for safe sprout production, and chemical disinfection of seed prior to sprouting. However, guidelines and best practices are only as good as their implementation. The consumption of raw sprouts is considered high-risk, especially for young, elderly and immuno-compromised persons (FDA, 2009).

Sol Erdozain, now a graduate student in psychology at Kansas State University, took the lead on this one. Kevin Allen, now a prof-type and hockey goon at the University of British Columbia who used to take great pleasure firing pucks off my head – even though he also a goalie himself – weighed in with his microbiology expertise, Katija Morley (nee Blaine) made our arguments more coherent, and I pestered everyone. Because I should have published something like this 12 years ago.

amy.sprouts.guelph.05From November 2010 into 2011, an outbreak linked to raw sprouts in the U.S. and involving sandwich franchise Jimmy John’s sickened 140 people. This was the third sprout related outbreak involving this franchise, yet the owner of the Montana Jimmy John’s outlet, Dan Stevens, expressed confidence in his sprouts claiming that because the sprouts were locally grown they would not be contaminated. By the end of December 2010 a sprout supplier, Tiny Greens Farm, was implicated in the outbreak. Jimmy John’s owner, John Liautaud, responded by stating the sandwich chain would replace alfalfa sprouts with clover sprouts since they were allegedly easier to clean. However, a week earlier a separate outbreak had been identified in Washington and Oregon in which eight people were infected with Salmonella after eating sandwiches containing clover sprouts from a Jimmy John’s restaurant. This retailer was apparently not aware of the risks associated with sprouts, or even outbreaks associated with his franchisees.

In late December, 2011, less than one year after making the switch to clover sprouts, Jimmy John’s was linked to another sprout related outbreak, this time it was E.coli O26 in clover sprouts. In February 2012, sandwich franchise Jimmy John’s announced they were permanently removing raw clover sprouts from their menus. As of April 2012, the outbreak had affected 29 people across 11 states. Founder and chief executive, John Liautaud, attempted to appease upset customers through Facebook stating, “a lot of folks dig my sprouts, but I will only serve the best of the best. Sprouts were inconsistent and inconsistency does not equal the best.” He also informed them the franchise was testing snow pea shoots in a Campaign, Illinois store, although there is no mention regarding the “consistency” or safety of this choice.

Despite the frequent need for sprout-based risk communication, messaging with industry and public stakeholders has been limited in effectiveness. In spite of widespread media coverage of sprout-related outbreaks, improved production guidelines, and public health enforcement actions, awareness of risk remains low. Producers, food service and government agencies need to provide consistent, evidence-based messages and, more importantly, actions. Information regarding sprout-related risks and food safety concerns should be available and accurately presented to producers, retailers and consumers in a manner that relies on scientific data and clear communications.

Abstract

Nutritional and perceived health benefits have contributed to the increasing popularity of raw sprouted seed products. In the past two decades, sprouted seeds have been a recurring food safety concern, with at least 55 documented foodborne outbreaks affecting more than 15,000 people. A compilation of selected publications was used to yield an analysis of the evolving safety and risk communication related to raw sprouts, including microbiological safety, efforts to improve production practices, and effectiveness of communication prior to, during, and after sprout-related outbreaks. Scientific investigation and media coverage of sprout-related outbreaks has led to improved production guidelines and public health enforcement actions, yet continued outbreaks call into question the effectiveness of risk management strategies and producer compliance. Raw sprouts remain a high-risk product and avoidance or thorough cooking are the only ways that consumers can reduce risk; even thorough cooking messages fail to acknowledge the risk of cross-contamination. Risk communication messages have been inconsistent over time with Canadian and U.S. governments finally aligning their messages in the past five years, telling consumers to avoid sprouts. Yet consumer and industry awareness of risk remains low. To minimize health risks linked to the consumption of sprout products, local and national public health agencies, restaurants, retailers and producers need validated, consistent and repeated risk messaging through a variety of sources.

USDA overhauls decades-old poultry inspections

Inspections and audits are never enough, because most of us don’t have bacteria-sensing goggles. But, given the tax dollars shelled out for inspections by the U.S. Department of Agriculture, they can do better.

pinto.explosiveThe Obama administration is overhauling poultry plant inspections for the first time in more than 50 years, a move it says could result in 5,000 fewer foodborne illnesses each year.

Final rules announced Thursday would reduce the number of government poultry inspectors. But those who remain will focus more on food safety than on quality, requiring them to pull more birds off the line for closer inspections and encouraging more testing for pathogens. More inspectors would check the facilities to make sure they are clean.

The changes would be voluntary, but many of the country’s largest poultry companies are expected to opt in. The chicken and turkey industries swiftly praised the new rules, saying they would modernize their business.

Federal law requires that government inspectors be present in poultry processing plants. Right now, many USDA inspectors stand in one place on the production line and check for visual defects. This doesn’t do much to ensure the birds are safe to eat, since common poultry pathogens like salmonella and campylobacter are invisible.

The new rules would better train inspectors to find hazards in the plant and would require all companies — whether they opt in or not — to do additional testing for pathogens.

Food safety inspections and audits are never enough, in China and elsewhere

When inspectors visited Shanghai Husi Food Co Ltd earlier this summer, the production line at the plant now at the centre of an international food scandal appeared in good order, with fresh meat being handled by properly-attired workers and supervisors keeping a watchful eye over the process.

audit.checklistHowever, if they had arrived unannounced a day before, they would have found piles of blue plastic bags filled with out-of-date meat stacked around the factory floor, a worker at the facility told Reuters, adding the old meat was often added back into the mix to boost production and cut costs.

“The next day, that meat just disappeared – someone must have disposed of it. The manager said it was an inspection,” said the worker, who wasn’t authorised to talk to the media and so didn’t want to be named.

On July 20, following an undercover local TV report that alleged workers used expired meat and doctored food production dates, regulators closed the factory, which is part of OSI Group LLC, a U.S. food supplier. Police have detained five people including Shanghai Husi’s head and quality manager.

The scandal – which has hit mainly big foreign fast-food brands including McDonald’s Corp and Yum Brands Inc, which owns the KFC and Pizza Hut chains – underlines the challenges facing inspectors in China’s fast-growing and sprawling food industry. China is Yum’s biggest market and McDonald’s third largest by outlets.

Behind the thousands of brightly-lit restaurants offering what Chinese consumers see as better quality food lie supply chains that rely on an army of poorly regulated and inadequately audited processing plants. Yum has around 650 suppliers in China alone.

China’s government has struggled to restore confidence in its $1 trillion food processing industry since six infants died in 2008 after drinking adulterated milk. The head of China’s Food and Drug Administration told the China Daily this week that the food safety situation “remains severe” and the existing oversight system “is not effective.”

Is that an African or European swallow? Can’t kill all the birds, but can manage risk

Zoonotic enteric pathogenic bacteria can live in the intestinal tract of birds and can be transmitted to food animals or humans via fecal contact. In the present study, cecal samples were collected from 376 migratory birds from species birdsfilm460often associated with cattle during the fall migration in the Central Flyway of the United States. Brown-headed cowbirds (n=309, Molothrus ater), common grackles (n=51, Quiscalus quiscula), and cattle egrets (n=12, Bubulcus ibis) contained foodborne pathogenic bacteria in their ceca. Salmonella enterica was isolated from 14.9% of all samples, and Escherichia coli O157:H7 from 3.7%. Salmonella serotypes isolated included the following: Muenster, Montevideo, and Typhimurium.

Our data suggest that migratory birds associated with cattle could be a vector for zoonotic enteric pathogenic bacteria to be disseminated across long distances.’

Nosestretcher alert: reusable grocery bags are a direct threat to public health

Brad Gertsman, lawyer, lobbyist and co-founder of the New York Association of Grocery Stores writes in a New York Post op-ed that reusable grocery bags are a health risk.

The irony here is that the bag fee poses a direct threat to New Yorkers’ health.

Problem is, reusable bags play a significant role in food contamination unless they’re properly washed on a regular basis — something people rarely do.

In a public-health study done at the University of Arizona, researchers found that only 3 percent of shoppers with multi-use bags said they regularly washed them. The same study found bacteria in 99 percent. Half carried coliform bacteria, and 8 percent carried E. coli — an indicator of fecal contamination.

A study by the Centers for Disease Control verified the dangers: “Reusable bags, if not properly washed between uses, create the potential for cross-contamination of foods.

“This potential exists when raw meat products and foods traditionally eaten uncooked (fruits and vegetables) are carried in the same bags, either together or between uses. This risk can be increased by the growth of bacteria in the bags.”

The paper Gertsman cites study showed that generic E. coli can be floating around in bags, recoverable in seven of the 84 bags in the study but can it be (or is it likely) to be transferred to any ready-to-eat foods, or somehow to food contact surfaces in the home? Just because the bacteria might be there, doesn’t mean it can contaminate a ready-to-eat food. No one has presented data to support that. In a cross-contamination event there is a dilution effect when it comes to transfer and it needs some sort of matrix to facilitate the transfer.

A couple of years later I exchanged emails with the insightful and entertaining Bill Keene (who is missed), who had investigated a norovirus outbreak that had been linked to storing foods in reusable bags in a hotel restroom. Keene confirmed that the type of bag was irrelevant: “This story had nothing to do with disposable bags, reusable bags, or anything similar. It is about how when norovirus-infected people vomit, they shower their surroundings with an invisible fog of viruses—viruses that can later infect people who have contact with those surroundings and their fomites. In this case these were was a reusable bag and its contents—sealed packages of Oreos, Sun Chips, and grapes— but it could just have easily been a disposable plastic bag, a paper bag, a cardboard box, the flush handle on the toilet, the sink, the floor, or the nearby countertops.”

We keep our reusable bags dry; wash them every couple of weeks and use one-use bags for raw meat.

Two cases of botulism in Ohio investigated, may be linked to California pesto

Mrs. Kalisz, my grade 7 and 8 family studies teacher warned of the dangers of botulism by showing our class a bulging can of beans (which she kept in a ziplock bag). What I took away from that story was to never buy or use dented cans (although that practice can be debated). Botulism from commercially canned foods has been pretty rare in North America since the 1970s with only a couple of cases in 40+ years.newpesto72_large

The first case of food-related botulism recorded in the medical literature occurred in Germany in 1735 and was traced to uncooked fermented blood sausage. Food safety history guru (and pretty decent margarita recipe developer) Carl Custer pointed out in an IAFP workshop that botulism concerns (and regulatory responses) go back further than that. In the 10th century, Emperor Leo VI of Byzantium prohibited the manufacture of blood sausage because of repeated illnesses leaving folks paralyzed and dying not too long after exposure. Botulism (derived from botulus, the latin word for sausage) is pretty nasty.

The spores, found commonly in soil, germinate and outgrow in anaerobic conditions (like partially-fermented sausages, under processed canned food, seafood and foil-wrapped baked potatoes) resulting in vegetative cells. A byproduct of the cells’ multiplication is the toxin.

According to Imperial Valley News, a pesto product may be linked to two cases of botulism in Ohio and as a result health officials from California are warning consumers to avoid VR Green Farms jarred food.  The pesto (and other processed products don’t seem to be available from their online store anymore).

California Department of Public Health (CDPH) Director Dr. Ron Chapman and state health officer warned consumers today not to eat VR Green Farms jarred food products because they may have been improperly produced, making them susceptible to contamination with Clostridium botulinum.

Ingestion of botulism toxin from improperly processed jarred and canned foods may lead to serious illness and death. CDPH is coordinating with the US Food and Drug Administration and the Ohio Department of Health in the investigation of two cases of suspected food-borne botulism infections that may be associated with consumption of the firm’s Pine Nut Basil Pesto.

VR Green Farms of San Clemente, California, is voluntarily recalling the following varieties of jarred food products: Pine Nut Basil Pesto, Pickled Farm Mix, Old World Tomato Sauce, Sundried Tomatoes in Olive Oil, Tuscan Grilling Sauce, and Pasta Sauce. These food products were sold under the VR Farms label and packaged in Mason-style glass jars with screw-on metal lids. The product labels do not include any coding or “use by” dates. Photographs can be found on Recalled Product Photos Page. The products were sold at the VR Green Farms stand in San Clemente, California and via the Internet to consumers throughout the United States.

 

Salmonella prompts hamburger recall in Sweden

Swedish supermarket chain Coop has recalled its in-store brand of fresh hamburger meat after a routine check revealed traces of salmonella in the meat.

salm.coop.swedenCoop halted sales of its hamburger meat on Wednesday evening. The recall applies to Coop Hamburger 10×113 grammes, 4×113 grammes and 2×113 grammes, Coop Miniburger 8×56,5 grammes and Coop Megaburger 2×170 grammes.

The affected meat has been sold throughout Sweden and has a best-before date of August 5th, 2014.

Except last year at the Ekka, the equivalent of a U.S. state fair, at least 50 people were stricken with E. coli O157 from the animal contact in the petting zoo.

There has been no public follow up, no reference to what is being done to improve the situation this year, and no chance we’ll be attending.

Educational events encouraging human–animal interaction include the risk of zoonotic disease transmission. It is estimated that 14% of all disease in the US caused by Campylobacter spp., Cryptosporidium spp., Shiga toxin-producing Escherichia coli (STEC) O157, non-O157 STECs, Listeria monocytogenes, nontyphoidal Salmonella enterica and Yersinia enterocolitica were attributable to animal contact. This article reviews best practices for organizing events where human–animal interactions are encouraged, with the objective of lowering the risk of zoonotic disease transmission.

E. coli O157 strikes UK boy in 2012; specialist recalls little Bo’s ‘darkest days’

Ian Ramage, consultant nephrologist at Yorkhill Children’s Hospital in Glasgow, has treated Bo since he first arrived from Aberdeenshire in May 2012.

Mr Ramage said: “I think the darkest days were not the times when he was most unwell, but when we realised that he would pull through and that he would be blind and require a stoma.

“All our energies had gone into keeping him alive, then we realised the burden his health would have on him and on Lucy.”

He said most children make a full recovery from E. coli, but added that those who do not generally suffer renal failure – as Bo did – and require a kidney transplant

However, the state of Bo’s bowel, which was left with five holes in it after being attacked by the bacteria, further complicated his case.

Meanwhile, new guidelines on managing the risks of E.coli have been hailed as a victory for common sense.
